Cooking oil fume exposure and Lung-RADS distribution among school cafeteria workers of South Korea

Background

Cooking oil fumes (COFs) from cooking with hot oil may contribute to the pathogenesis of lung cancer. Since 2021, occupational lung cancer for individual cafeteria workers has been recognized in South Korea. In this study, we aimed to identify the distribution of lung-imaging reporting and data system (Lung-RADS) among cafeteria workers and to determine factors related to Lung-RADS distribution.

Methods

We included 203 female participants who underwent low-dose computed tomography (LDCT) screening at a university hospital and examined the following variables: age, smoking status, second-hand smoke, height, weight, and years of service, mask use, cooking time, heat source, and ventilation. We divided all participants into culinary and non-culinary workers. Binomial logistic regression was conducted to determine the risk factors on LDCT of Category ≥ 3, separately for the overall group and the culinary group.

Results

In this study, Lung-RADS-positive occurred in 17 (8.4%) individuals, all of whom were culinary workers. Binary logistic regression analyses were performed and no variables were found to have a significant impact on Lung-RADS results. In the subgroup analysis, the Lung-RADS-positive, and -negative groups differed only in ventilation. Binary logistic regression showed that the adjusted odds ratio (aOR) of the Lung-RADS-positive group for inappropriate ventilation at the workplace was 14.89 (95% confidence interval [CI]: 3.296–67.231) compared to appropriate ventilation as the reference, and the aOR for electric appliances at home was 4.59 (95% CI: 1.061–19.890) using liquid fuel as the reference.

Conclusions

The rate of Lung-RADS-positive was significantly higher among culinary workers who performed actual cooking tasks than among nonculinary workers. In addition, appropriate ventilation at the workplace made the LDCT results differ. More research is needed to identify factors that might influence LDCT findings among culinary workers, including those in other occupations.

한국 학교 급식소 종사자의 조리흄 노출과 Lung-RADS 분포
목적
2021년부터 한국에서는 개별 학교 급식 종사자의 폐암 산업재해가 인정되고 있다. 본 연구에서는 급식 종사자들의 Lung-imaging reporting and data system ( Lung-RADS ) 분포를 확인하고, Lung-RADS 분포와 관련된 요인을 탐색해보고자 한다.
방법
2020년도에 울산대학병원에서 저선량 CT를 촬영한 여성 참가자 중 203명을 대상으로 나이, 흡연 여부, 간접 흡연 여부, 키, 체중, 근무 년수, 마스크 착용 여부, 조리 시간, 열원, 환기방식 등의 변수를 조사하였다. 참가자를 조리직, 비조리직으로 분류하였다. 범주 3이상에 대한 영향을 확인하기 위해 이분형 로지스틱을 전체집단 조리직 집단에 나누어 실시하였다.
결과
Lung-RADS 양성은 17 (8.4%)명에게서 발생했고 모두 조리직에 해당했다. 이분형 로지스틱 회귀분석을 시행한 결과 전체집단에서 Lung-RADS 결과에 유의미한 영향을 주는 변수는 확인할 수 없었다. Subgroup 분석에서 Lung-RADS 양성, 음성군은 직장 내 환기방식에서만 차이가 있었다. 이분형 로지스틱 회귀분석 결과 직장에서 환기를 철저히 한 것을 참고치로 했을 때 부적절한 환기에 대한 Lung-RADS 양성의 오드비가 14.89이고 가정에서 액화연료 사용을 참고치로 했을 때 전기 요리기구 사용의 오드비는 4.59이였다.
결론
실제 조리업무를 시행하는 조리직의 경우 비조리직에 비해 Lung-RADS 양성 비율이 유의하게 높았다. 또한 조리실 환경에서 환기를 철저하게 하는 것이 Lung-RADS 소견에 영향을 주었다.

Association between shift work and serum homocysteine level in female electronic manufacturing services workers

Background

Shift work has been shown to increase the risk of cardiovascular disease (CVD) based on several evidences. The classic risk factors of CVD include age, hypertension, smoking, obesity and diabetes. Recently, the serum homocysteine level has been reported to be a valuable indicator of CVD risk. This study aimed to determine the variation in serum homocysteine level as a cardiovascular risk indicator among female workers according to shift work.

Methods

The data of regular health examination of workers at an electronic manufacturing services company in Yeongnam region, South Korea in 2019 were examined in this study. The investigation was based on a cross-sectional study conducted on 697 female workers (199 day workers and 498 shift workers). The sociodemographic and biochemical characteristics were compared between day workers and shift workers. Through a logistic regression analysis, the odds ratio (OR) of the increased serum homocysteine level in relation to shift work was determined.

Results

Compared to female day workers, female shift workers showed significantly higher level of serum homocysteine (8.85 ± 2.16 vs. 9.42 ± 2.04 μmol/mL; p = 0.001). The OR of day workers against shift workers was 1.81 (95% confidence interval [CI]: 1.25–2.63). With the adjustment of variables that may influence the level of serum homocysteine, the adjusted OR was 1.68 (95% CI: 1.09–2.60).

Conclusions

The serum homocysteine level was significantly higher in shift workers than in day workers. It is thus likely to be a useful predictor of CVD in shift workers.

전자제조서비스업 회사의 여성 근로자에서 교대근무와 혈청 호모시스테인 수준과의 연관성
목적
교대근무가 심혈관계질환의 발병위험도를 증가시킨다는 여러 증거들이 있다. 심혈관계질환의 고전적 위험인자로는 나이, 고혈압, 흡연, 비만, 이상지질, 당뇨병 등이 있으며, 최근 혈청 호모시스테인 수치는 심혈관계 질환의 위험도 지표로 유용한 것으로 알려지고 있다. 본 연구에서는 여성 근로자에서 교대근무 여부에 따라 심혈관계 질환의 위험도 지표인 혈청 호모시스테인의 수준 차이가 있는지를 알아보고자 하였다.
방법
본 연구는 2019년 영남지방의 한 전자제조서비스업회사 근로자의 정기건강검진 자료를 이용하여 수행하였다. 총 697명의 여성근로자(주간근무자 199명, 교대근무자 498명)의 데이터로 단면연구를 수행하였다. 주간 근무자와 교대근무자의 인구사회학적, 생화학적 특성을 비교하였다. 로지스틱 회귀분석을 사용하여 교대근무와 상승된 혈청 호모시스테인 수치의 교차비를 조사하였다.
결과
여성 주간 근로자와 비교하여 여성 교대근무자의 혈청 호모시스테인 수치가 통계적으로 유의하게 더 높았다. (8.85 ± 2.16 vs 9.42 ± 2.04 μmol/ml ; p=0.001). 교대근무자에 비해 주간 근무자의 교차비는 1.81(95% CI : 1.25-2.63) 였으며, 혈청 호모시스테인 수치에 영향을 미칠 수 있는 변수를 보정한 후, 수정된 OR은 1.68(95% CI : 1.09-2.60) 이었다.
결론
혈청 호모시스테인 수치는 주간 근무자에 비해 교대근무자에서 통계적으로 유의하게 높았다. 교대근로자의 심혈관계 질환 발병의 예측에 있어 혈청 호모시스테인 수치가 유용하겠다.

Factors related to femoral bowing among Korean female farmers: a cross-sectional study

Background

Female farmers have a high prevalence of knee osteoarthritis (KOA) in South Korea. Femoral bowing has been reported to be related to KOA by increasing load on the mechanical axis. This study aimed to investigate factors related to femoral bowing in Korean female farmers.

Methods

We analyzed the legs of 264 female farmers registered with the Korea farmers' knee cohort of Jeonnam Center for Farmers' Safety and Health. A structured questionnaire was used to determine sociodemographic variables, agricultural career, cumulative squatting working time (CSWT), and cumulative heavy lifting working time. Femoral bone density was measured and Kellgren-Lawrence (KL) grades were obtained from the knee radiographs. Mechanical axis angle (MAA), femoral bowing angle (FBA), anatomical lateral distal femoral angle (aLDFA), anatomical medial proximal tibial angle (aMPTA), and condylar-plateau angle (CPA) were measured. We examined the relationship between the FBA and related factors by using multiple linear regression.

Results

The proportion of individuals with radiographic KOA (≥ KL grade 2) in this study was 37.9%. As KL grades increased, MAA, FBA, and CPA increased, whereas aLDFA and aMPTA decreased. FBA increased with age. Multiple linear regression analyses using FBA as a dependent variable showed relationship with higher age, lower height, higher BMI, lower bone mineral density, longer CSWT, and longer agricultural careers.

Conclusions

The results of this study suggest that external factors related to agricultural work in female farmers was associated with femoral bowing, in addition to internal factors such as age, bone density, height, and obesity.

Pregnancy, childbirth, and puerperium outcomes in female firefighters in Korea

Background

Female firefighters are exposed to hazardous environmental (chemical and physical) and working (shift work, psychological, and ergonomic factors) conditions that have reported or are suspected of adverse effects on reproductive health. However, no previous studies have reported on pregnancy, childbirth, and puerperium outcomes (PCPOs) in female firefighters.

Methods

The present study compared hospital admissions for PCPOs, based on 10th revision of the International Statistical Classification of Diseases and Related Health Problems (ICD-10) codes, among Korean female firefighters with those of the general Korean population. Standardized admission ratios (SARs) and their 95% confidence intervals (CIs) were calculated.

Results

The study population included 1,766 female firefighters. Total follow-up duration was 9,659 person-years. Compared to the general female population, the female firefighters' SARs were higher in all admissions for PCPOs (SAR, 1.92; 95% CI: 1.79–2.05); pregnancy and abortive outcomes (SAR, 1.56; 95% CI: 1.12–2.12); other maternal disorders predominantly related to pregnancy (SAR, 2.65; 95% CI: 1.99–3.46); maternal care related to the fetus, amniotic cavity, and possible delivery problems (SAR, 2.13; 95% CI: 1.74–2.57); labor and delivery complications (SAR, 1.55; 95% CI: 1.15–2.06); delivery (SAR, 1.94; 95% CI: 1.80–2.08); and complications predominantly related to puerperium (SAR, 4.68; 95% CI: 2.02–9.23).

Conclusion

The results of this study showed high SARs in all and specific subcategories of PCPOs in female firefighters.

The effect of shift work on high sensitivity C-reactive protein level among female workers

Background

This study assessed the association between shift work and high-sensitivity C-reactive protein (hs-CRP) level, a risk factor for cardiovascular disease (CVD), in female workers in electronics manufacturing services (EMS).

Methods

Female EMS workers who received special medical examinations for workers in Gyeongnam, Korea between January 2017 and December 2017 were enrolled in this study. Their age, marital status, education level, alcohol consumption, smoking habit, regular exercise, quality of sleep, work stress, and depression were investigated, and blood tests were conducted. The t- and χ2 tests were conducted to compare the general and biochemical characteristics between daytime and shift worker groups. Age-adjusted partial correlation analysis was performed to examine the linear relationship between hs-CRP level and other risk factors for CVDs. In addition, the difference in hs-CRP levels according to work schedule was analyzed by ANCOVA after adjusting for variables that could affect the hs-CRP level.

Results

Although the average hs-CRP levels did not differ significantly between daytime and shift workers (0.92 ± 1.87 and 1.07 ± 2.20 mg/dL, respectively), shift workers tended to show a higher hs-CRP level (p = 0.067). After adjusting for variables that can affect the hs-CRP level, the estimated average hs-CRP level was significantly higher in shift workers (1.325 ± 0.156 mg/dL) than that in daytime workers (0.652 ± 0.350 mg/dL) (p = 0.003).

Conclusions

The results of this study identified a relationship between shift work and hs-CRP level increase in women. Because multiple studies have reported associations between increased hs-CRP and CVD, follow-up of hs-CRP may help early detection of CVD in shift workers.

The association between long working hours and marital status change: middle-aged and educated Korean in 2014–2015

Background

We investigated the association between long workhours and marital status change from married to divorced or separated status that might have bad health effects.

Methods

A total of 40,654 participants with married status in 2014 were followed up in 2015. Weekly workhours were categorized into four groups: ≤ 40, 41–52, 53–60, and > 60 hours per week. Univariate and multivariate logistic regression analyses were performed to determine the relationship between groups of workhours and marital status change after adjusting for age, total monthly household income, working type, and depression with sex stratification.

Results

The study populations consisted of 8,346 (20.5%) females and 32,308 (79.5%) males. Odd ratios (ORs) of marital status change for females working for more than 60 hours per week was 4.26 (95% confidence interval [CI]: 1.25–14.5), when working less than or equal to 40 hours per week was used as reference in the crude model. ORs of working more than 60 hours per week was 4.57 (95% CI: 1.02–20.5) in female workers when considering age, total household earning per month, working type of daytime, and depression in a dose-response manner. However, for male workers, long workhours were not significantly related to change of marriage status.

Conclusions

Long workhours for more than 60 hours per week had significantly higher risk of divorce or separation in females, but not in males. Further follow-up studies are needed to evaluate long term effects of long workhours on divorce risk.

Background

Most of the school meal service workers in Korea are middle-aged individuals. They have high workload, which increases their incidence of musculoskeletal disorders. This study aimed to evaluate the prevalence and risk factors of subjective musculoskeletal symptoms, presumptive diagnosis, medical care use, and sick leave among female school meal workers.

Methods

We analyzed the results of musculoskeletal disease screening of 1581 female school meal workers. The screening consisted of self-administered questionnaire, history taking by occupational physicians, and physical examination. The prevalence of subjective musculoskeletal symptoms, presumptive diagnosis after initial examination by occupational physicians, use of medical care for more than 7 days, and sick leave due to musculoskeletal diseases during the past year were evaluated in this study. The relative risk of four outcome indicators of musculoskeletal disorders was compared with respect to potential factors, such as age, subjective physical loading, present illness, injury experience, and type of school, using log-binomial regression.

Results

The prevalence of subjective musculoskeletal symptom was 79.6%; presumptive diagnosis, 47.6%; hospital visits over 7 days, 36.4%; and sick leave, 7.3%. The relative risk of musculoskeletal symptoms by age (≥50 years vs < 50 years) was 1.04 (95% confidence interval (CI): 1.00–1.09); presumptive diagnosis of musculoskeletal disease, 1.17 (95% CI: 1.06–1.30); hospital visits over 7 days, 1.26 (95% CI: 0.85–1.85); and sick leave, 1.17 (95% CI: 1.02–1.34). The relative risk of musculoskeletal symptoms due to subjective physical loading (very hard vs low) was 1.45 (95% CI: 1.33–1.58); presumptive diagnosis, 2.92 (95% CI: 2.25); hospital visits over 7 days, 1.91 (95% CI: 1.02–3.59); and sick leave, 2.11 (95% CI: 1.63–2.74).

Conclusions

Subjective physical loading was a more important factor in musculoskeletal disorders than the age of female school meal workers.

Effects of shift work on abdominal obesity among 20–39-year-old female nurses: a 5-year retrospective longitudinal study

Background

This study aimed to investigate the effects of shift work on abdominal obesity among young and middle-aged female nurses during a 5-year retrospective study.

Methods

This retrospective study included female nurses (20–39 years old) who worked at a university hospital in Korea and had available health screening results from 2010–2015. Among 2,611 employees, 934 healthy 20–39-year-old female nurses were identified, and data regarding their demographic information (age and date of employment), waist circumferences (WC), and lifestyle factors (alcohol and exercise) were obtained. Abdominal obesity was defined as a WC of ≥80 cm, based on the World Health Organization’s Asia-West Pacific standard in 2000. The mean WC change from baseline was analyzed using the paired t test, and the association between shift work and abdominal obesity was analyzed using the generalized estimating equation.

Results

Compared to all day workers (both age groups), the 20–29-year-old nurses did not exhibit significant changes in WC at each follow-up. However, among the 30–39-year-old nurses, shift workers exhibited a significant change in WC (vs. baseline) during years 4 and 5, compared to day workers. After adjusting for effective confounders and stratifying the participants according to age, the 20–29-year-old nurses exhibited an odds ratio of 3.21 (95 % confidence interval: 1.29–7.98) for shift work-associated obesity, although the odds ratio for the 30–39-year-old nurses was not statistically significant.

Conclusion

In the study population, shift work was associated with a significant change in mean WC among 30–39-year-old nurses, and the shift work-associated risk of abdominal obesity was significant among 20–29-year-old nurses. These results indicate that shift work may influence abdominal obesity differently in 20–29-year-old and 30–39-year-old female nurses.

The Association between Shift Work and the Metabolic Syndrome in Female Workers

Objective

This study aimed to determine identify any association between shift work and the metabolic syndrome by comparing the prevalence rates of the metabolic syndrome in shift work groups and daytime work groups for female workers.

Methods

Based on data from health examinations carried out from April to December of 2012, we selected as our subjects 254 female workers from the Daegu area Dyeing Industrial Complex. We diagnosed the metabolic syndrome using the examination results, and information about age, whether or not they did shift work, job type, smoking habits, drinking habits, exercise habits, and past medical history was collected through self-administered questionnaire surveys and face-to-face interviews. The variables found in a univariate analysis to be significant in the occurrence of the metabolic syndrome - age, drinking habits, exercise habits, and shift work - were included in a logistic regression analysis of the risk of the metabolic syndrome for female workers.

Results

The prevalence rates of the metabolic syndrome for the total group of study subjects was 11.8%, for daytime workers was 2.8%, and for shift workers was 15.3%. A logistic regression analysis of the odds of the metabolic syndrome for female workers was conducted that included factors associated with the occurrence of the metabolic syndrome: age, drinking habits, exercise habits, and shift work. The results revealed that the odds ratio of the metabolic syndrome in the shift work group, 6.30 (95% CI 1.24-32.15), was significantly higher when compared with the daytime work group.

Conclusion

Shift work appears to have an association with the metabolic syndrome in female workers. Accordingly, we believe that the attention of government agencies and business owners is needed together with the individual practice of health behaviors to manage the metabolic syndrome for the prevention of cardiovascular disease in female shift workers.

The Problem Drinking and Related Factors in Female Firefighters

OBJECTIVES
This study was conducted to investigate problem drinking and analyze factors associated with alcohol use among female firefighters, as well as to provide useful data for management of alcohol use by female firefighters.
METHODS
The study includeed 1,587 female firefighters from throught Koea. Data were collected from June to July in 2008 using self-administered questionnaires including a general questionnaire, work related questionnaire, the Alcohol Use Disorders Identification Test (AUDIT), the Korean Occupational Stress Scale Short Form (KOSS-26), and the Center for Epidemiologic Studies Depression Scale (CESD). Among 1,263 female firefighters (75.6%) surveyed, the results of 1,084 qualified questionnaires (70.5%) were analyzed using the chi-square test, Fisher's exact test, and multiple logistic regression.
RESULTS
The prevalence of problem drinking (AUDIT score > or =8) in female firefighters was 12.5%. Multiple logistic analysis showed that being single, smoking and depression were correlated with problem drinking. Single female firefighter, present smokers and individuals positive for symptoms of depression had OR values of 2.11, 17.58 and 1.76, respectively.
CONCLUSIONS
Three factors were found to influnce the occurrence of problem drinking. Accordingly, smoking related education, counseling to cope effectively with depression and health promotion programs that consider psychological effects of job stress are needed to reduce work related factors that lead to problem drinking by female firefighters.

Relationship of Shift Work to Cardiovascular and Gastrointestinal Symptoms in Korean Female Workers

OBJECTIVES
We wanted to investigate the relationship of shift work to the cardiovascular and gastrointestinal symptoms of Korean female workers.
METHODS
The data was analyzed from a cross-sectional survey of 1,875 Korean female workers. Information was obtained about the shift work, the cardiovascular and gastrointestinal symptoms and the background characteristics. Multiple regression analysis was used to test for the relationship between the cardiovascular and gastrointestinal symptoms and the shift work variables, while controlling for the covariates.
RESULTS
Shift work was more common for blue-collar workers than for office workers. A higher proportion of current smokers and drinkers was observed for shift workers than for day workers. A full-time job was more popular for shift workers than for day workers, while more day workers worked over 8 hours per day than did the shift workers. The mean scores of the cardiovascular and gastrointestinal symptoms were significantly higher for shift workers than for day workers. Shift work was associated with 0.54 more cardiovascular symptoms and with 0.38 more gastrointestinal symptoms on a multiple regression analysis (p=0.001; p=0.020, respectively).
CONCLUSIONS
Our findings suggest that shift work may increase the risks for cardiovascular and gastrointestinal disturbances. Therefore, preventive strategies are needed to control the negative effects of shift work in the workplace.

The Effect of Breath Hold Diving on Bone Mineral Density of Women Fishery Diver

OBJECTIVES
The aim of this study was to determine the effect of breathhold diving on female diver's bone mineral density (BMD).
METHODS
A cross-sectional observational study was carried out utilizing a health checkup of divers and controls at a hospital in Jeju city, Korea. Female divers (n=61) were matched with non-diver controls (n=61) by age, weight and postmenopausal year. BMD of the proximal femur(femoral neck, trochanter and Ward's triangle) was assessed by dual-energy X-ray absorptiometry (DEXA).
RESULTS
The average diving duration of the female divers was 34+/-13 years. The BMD of the divers was higher than that of the controls in the femur neck and trochanter (p<0.05). On correlations analysis, the status of menopause, age, body weight and age at menarche were significantly correlated with proximal femur BMD in the divers (p<0.05). On multiple regression analysis, age and body weight were predictors of proximal femur BMD in the divers. On linear regression analysis of proximal femur BMD according to age in the divers and controls, regression coefficients of the divers were lower in all 3 areas of the proximal femur than the controls.
CONCLUSIONS
BMD of the divers was higher than that of the controls in the femur neck and trochanter. There was a tendency for BMD of the divers to decrease faster than that of the controls with increasing age.
