Particulate matter concentration effects on attention to environmental issues: a cross-sectional study among residents in Korea’s Pohang Industrial Complex

Background

With modernization, air pollution has become increasingly serious, and its effects on health have been revealed. As a result, public interest in environmental pollution has become critical for regulating air pollution. In our study, we aim to evaluate the impact of air pollution levels on public attention to environmental issues and examine whether awareness of the residential environment’s impact on health acts as a mediator in this relationship.

Methods

We conducted an analysis on 400 individuals surveyed in the preliminary feasibility study on adverse health effects in the Pohang Industrial Complex, to examine the relationship between particulate matter 2.5 (PM2.5) and attention to environmental issues. Logistic regression analysis was performed, and mediation analysis was used to determine whether awareness of the residential environment’s impact on health mediated the relationship.

Results

The logistic regression analysis results showed that PM2.5 levels were associated with attention to environmental issues (adjusted odds ratio [AOR]: 2.1; 95% confidence interval [CI]: 1.3–3.5; p = 0.003) and awareness of health impacts (AOR: 3.4; 95% CI: 1.6–7.1; p = 0.001). The PM2.5 levels showed 9.9% (95% CI: 5.4–14.0) increase in the prevalence of high attention to environmental issues, of which, only 1.0% (95% CI: 0.2–2.3) were mediated by health impact awareness. In the overall analysis, 10% of the total effect of PM2.5 on attention to environmental issues was mediated by health impact awareness.

Conclusions

According to this study’s results, there was a correlation between air pollution levels and attention to environmental issues. Awareness of the health impacts of air pollution partially mediated the effect of air pollution levels on attention to environmental issues. In future studies, it is recommended to identify other mediators to further understand this structure.

미세먼지(Particular matter)농도가 환경문제에 대한 관심에 미치는 영향 : 포항시 거주민을 대상으로 한 단면연구
목적
근현대화가 진행되어오면서 대기오염은 심각해지고 대기오염에 의한 건강영향이 밝혀지면서 대기오염을 규제하기위해 환경오염에 대한 대중의 관심도가 중요해졌다. 이번 연구에서는 대기오염 수치가 환경오염에 대한 관심도에 미치는 영향을 평가하고 그 구조로써 건강영향에 대한 인식이 매개체로써 작용하는지 탐구하고자 하였다.
방법
포항 산업단지 건강피해 예비타당성 위해소통 설문조사에서 파악된 400명에 대하여 분석을 시행하였다. PM2.5와 환경오염에 대한 관심의 관계에 대해서 보기위해 로지스틱 회귀분석을 시행하였고, 그 구조를 파악하기 위해 매개분석을 사용하여 건강영향에 대한 인식이 매개체로써 작용하는지 확인하였다.
결과
로지스틱 분석 결과 PM2.5 수치는 환경에 대한 관심과 관련이 있었고([AOR]: 2.1, 95% CI: 1.3, 3.5, P = 0.003), 건강영향에 대한 인식도와도 관련이 있었다(AOR: 3.4, 95% CI: 1.6, 7.1, P = 0.001). PM2.5수치는 전체 환경에 대한 관심도에 비교위험(%)이 9.9 (95 % CI: 5.4, 14.0)인데 전체에서 1.0 (95 % CI: 0.2, 2.3) 만이 건강영향 인식도에 의해 매개되는 비교위험이다. 전체 분석 결과에서 PM2.5가 환경에 대한 관심에 미치는 전체 영향의 10%가 건강영향 인식에 매개되었다.
결론
연구결과에 따르면 대기오염 수준은 환경오염에 대한 관심도와 관련이 있었다. 건강영향에 대한 인식은 대기오염수준이 환경오염에 대한 관심도에 미치는 영향의 일부를 매개하였다.

The effect of blood cadmium levels on hypertension in male firefighters in a metropolitan city

Background

This study investigated the effect of dispatch frequency on blood cadmium levels and the effect of blood cadmium levels on hypertension in male firefighters in a metropolitan city.

Methods

We conducted a retrospective longitudinal study of male firefighters who completed the regular health checkups, including a health examination survey and blood cadmium measurements. We followed them for 3 years. To investigate the effect of dispatch frequency on blood cadmium levels and the effect of blood cadmium levels on hypertension, we estimated the short-term (model 1) and long-term (model 2) effects of exposure and hypothesized a reversed causal pathway model (model 3) for sensitivity analysis. Sequential conditional mean models were fitted using generalized estimating equations, and the odds ratios (ORs) and the respective 95% confidence intervals (CIs) were calculated for hypertension for log-transformed (base 2) blood cadmium levels and quartiles.

Results

Using the lowest category of dispatch frequency as a reference, we observed that the highest category showed an increase in blood cadmium levels of 1.879 (95% CI: 0.673, 3.086) μg/dL and 0.708 (95% CI: 0.023, 1.394) μg/dL in models 2 and 3, respectively. In addition, we observed that doubling the blood cadmium level significantly increased the odds of hypertension in model 1 (OR: 1.772; 95% CI: 1.046, 3.003) and model 3 (OR: 4.288; 95% CI: 1.110, 16.554). Using the lowest quartile of blood cadmium levels as a reference, the highest quartile showed increased odds of hypertension in model 1 (OR: 2.968; 95% CI: 1.121, 7.861) and model 3 (OR: 33.468; 95% CI: 1.881, 595.500).

Conclusions

We found that dispatch frequency may affect blood cadmium levels in male firefighters, and high blood cadmium levels may influence hypertension in a dose-response manner.

대도시 남성 소방관에서의 혈중 카드뮴 농도가 고혈압에 미치는 영향
목적
본 연구에서는 대도시 남성 소방관에서 출동빈도가 혈중 카드뮴 농도에 미치는 영향과 혈중 카드뮴 수치가 고혈압에 미치는 영향을 조사하였다.
방법
남성 소방관들을 대상으로 설문조사와 혈중 카드뮴 농도 및 혈압 측정을 포함하는 건강검진을 실시하였다. 2015년부터 2017년까지 3년 간 추적하였으며, 최종 연구 대상자는 215명이었다. 출동빈도가 혈중 카드뮴 농도에 미치는 영향과 고혈압에 대한 혈중 카드뮴 농도의 영향을 조사하기 위해 단기간 노출 효과 (모델1), 장기간 노출 효과 (모델2), 역방향 노출 효과(모델3)를 가정하여 분석하였다. 순차적 조건부 평균 모델 (sequential conditional mean model)을 적용하였고, 일반화 추정 방정식 (generalized estimating equations)을 사용하여 오즈비와 95% 신뢰구간을 계산하였다.
결과
가장 낮은 사분위의 출동 빈도 그룹을 기준으로 가장 높은 사분위수 그룹은 모델 2와 모델3에서 혈중 카드뮴 수치가 각각 1.879 (95% CI 0.673-3.086) μg/dL, 0.708 (95% CI 0.023-1.394) μg/dL 증가하였다. 혈중 카드뮴 농도가 두 배로 증가할 때마다 모델 1과 3 (OR [95% CI] 1.772 [1.046–3.003], 4.288 [1.110–16.554])에서 고혈압 발생 위험이 증가한다는 것을 관찰했다. 혈중 카드뮴 농도가 가장 낮은 사분위수 그룹을 기준으로 할 때, 가장 높은 사분위수 그룹은 모델 1과 3(OR [95% CI] 2.968 [1.121–7.861], 33.468 [1.881–595.500])에서 고혈압 발생 위험이 증가하였다. 혈중 카드뮴 농도와 고혈압 발생 사이의 용량-반응 관계는 모든 시나리오에서 관찰되었다.
결론
본 연구에서는 출동 빈도가 혈중 카드뮴 농도에 영향을 미칠 수 있고, 높은 혈중 카드뮴 농도가 고혈압에 영향을 미칠 수 있다는 것을 발견했다. 다양한 연구모집단에 대해 카드뮴의 다양한 생체지표를 사용하여 추가 연구가 수행되어야 할 것이다. 또한 일반적으로 단일 중금속 노출보다는 복합 중금속 노출의 가능성이 높으므로, 향후 연구에서는 중금속의 상호작용에 대해 평가하여야 할 것이다.

Comparison of risk-assessment tools for cardio-cerebrovascular diseases (CVD) in male shipyard workers: a cross-sectional study

Background

Periodic revision of assessment tools is essential to ensure risk assessment reliability and validity. Despite the recent revision of the Korea Occupational Safety and Health Agency (KOSHA) 2018, there is no evidence showing that the revision is superior to other cardio-cerebrovascular diseases (CVDs) risk-assessment tools for workplace health management. We conducted a comparative analysis using the Framingham risk score (FRS) as a gold standard to identify the most relevant CVDs risk-assessment tool for workplace health management.

Methods

We included 4,460 shipyard workers who had undergone a workers' health examination during January–December 2016. Risk levels for CVDs were calculated based on the FRS, KOSHA 2013, KOSHA 2017, KOSHA 2018 (2 methods), National Health Screening Program health risk appraisal (NHS HRA) 2017, and NHS HRA 2018. Study participants were categorized into low-risk, moderate-risk, or high-risk groups. Sensitivity, specificity, correlation, and agreement of each risk-assessment tool were calculated compared with the FRS as a gold standard. For statistical analyses, Spearman's rank correlation coefficient and the linearly weighted kappa coefficient were calculated.

Results

Sensitivity of the risk assessments was highest in the KOSHA 2018 (health risk appraisal [HRA]). The FRS showed correlation coefficients of 0.354 with the KOSHA 2013, 0.396 with the KOSHA 2017, 0.386 with the KOSHA 2018, 0.505 with the KOSHA 2018 (HRA), 0.288 with the NHS HRA 2017, and 0.622 with the NHS HRA 2018. Kappa values, calculated to examine the agreement in relation to the KOSHA 2013, KOSHA 2017, KOSHA 2018, KOSHA 2018 (HRA), NHS HRA 2017, and NHS HRA 2018 with the FRS, were 0.268, 0.322, 0.352, 0.136, 0.221, and 0.559, respectively.

Conclusions

The NHS HRA 2018 risk calculation method is a useful risk-assessment tool for CVDs, but only when appropriate classification criteria are applied. In order to enhance the risk-group identification capability of the KOSHA guideline, we propose to apply the classification criteria set in this study based on the risk group definition of the 2018 Korean Society of Hypertension guidelines for the management of hypertension instead of the current classification criteria of the KOSHA 2018.

Changes of depression and job stress in workers after merger without downsizing

Background

Since the 1980s, restructuring, which includes downsizing, closures, mergers, and privatization, has expanded worldwide, and various studies have investigated its effect on health. However, previous studies have mainly focused on restructuring accompanied by massive lay-offs, and the effect of a merger on workers’ health is still controversial. This study aims to investigate changes in worker depression and job stress after a merger without downsizing, which is unusual in Korea.

Methods

Repeated surveys were done in April 2014, April 2015, and April 2016 involving the participation of 209 subjects. Participants were divided into two groups, which were comprised of blue-collar workers (104) and white-collar workers (105). Sociodemographic characteristics, including age, education level, job tenure, gender, marital status, smoking status, and alcohol consumption, were measured via a survey. To determine the level of depression, the Korean version of the Center for Epidemiologic Studies Depression Scale (CES-D) was employed, and to investigate job stress, the Korean Occupational Stress Scale-Short Form (KOSS-SF) was used. For statistical analyses, Pearson’s chi-square test, the Student’s t-test, and repeated measure analysis of variance (ANOVA) were performed.

Results

The results showed that depression (CES-D, F[2, 400] = 0.466, p = 0.628) was changed but without significance and job stress (KOSS-SF, F[1.899, 379.831] = 3.192, p = 0.045) were significantly different. The between-group difference in the CES-D score between the blue- and white-collar workers by survey administration time was not statistically significant (F = 0.316, p = 0.574). The interaction between the survey time and occupational group was also not statistically significant (F = 0.967, p = 0.381). The between-group difference in the KOSS-SF total score was not statistically significant (F = 1.132, p = 0.289), and the interaction between the survey administration time and occupational group was also not significant (F = 0.817, p = 0.437). In the job stress subgroup analyses Job insecurity and Lack of reward showed a significant difference by survey administration time.

Conclusion

This study showed that a merger without massive downsizing can cause negative health effects such as an changes in depression and increase in job stress. To improve the health of workers, both the immediate negative effects on health, and the long-term effects or their resolution over time should be considered prior to the merger.
