Effects of a supportive workplace environment on the success rate for smoking cessation camp

Background

This study was conducted to identify the success rate for smoking cessation over time after participation in a therapeutic smoking cessation camp, and to identify how participant characteristics, including a supportive workplace environment for smoking cessation (SWESC), affect the success rate for smoking cessation.

Methods

In all, 296 participants at smoking cessation camps in Ulsan between 2015 and 2020 were investigated. The success rates of smoking cessation after weeks 4, 6, 12, and 24 at camp were investigated. The participants were grouped as workers with an SWESC, and workers without an SWESC, and variables (age, education, household income, marital status, drinking, exercise, body mass index, morbidity, job, number of counseling sessions, cigarettes smoked per day and smoking initiation age) were investigated. Multiple logistic regression analysis was conducted at each time point. In addition, Cox regression analysis was performed to evaluate the variables affecting the success rate for smoking cessation over time.

Results

The smoking cessation success rate of workers with an SWESC at week 24 (90.7%) was higher than that for workers without an SWESC (60.5%). Multiple logistic regression was performed to determine the relationship between each variable and the success rates for smoking cessation at week 6, 12, and 24. SWESC was confirmed as significant (p < 0.05) variables for increased success rate for smoking cessation at all 3 time points. After adjusting for all variables, the Cox proportional hazards survival analysis showed a hazard ratio of 6.17 for SWESC (p < 0.001,; 95% confidence interval: 3.08–12.38).

Conclusions

At a professional treatment smoking cessation camp, participants with an SWESC showed a significantly higher success rate for smoking cessation. Supportive workplace environment for workers’ health is expected to be an important factor for smoking cessation projects as well as other health promotion projects at workplace.

직장의 금연에 대한 지지적 환경이 전문 치료형 금연 캠프의 금연성공률에 미치는 영향
목적
본 연구는 울산에서 진행된 전문 치료형 금연 캠프에 참가한 후 시간 경과에 따른 금연 성공률을 알아보고, 직장의 금연에 대한 지지적 환경 (SWESC: supportive workplace environment for smoking cessation)을 포함한 각 참가자의 특성이 금연 성공률에 어떤 영향을 미치는지 알아보기 위해 실시되었다.
방법
2015-2020년 울산 지역 금연 캠프 참가자 455명을 대상으로 자가 보고, 호기 일산화탄소, 소변 코티닌 수치를 이용하여 캠프 후 4주, 6주, 12주, 24주차 금연 성공률을 조사하였다. 캠프 참가자를 SWESC가 있는 근로자 및 SWESC가 없는 근로자로 분류하여 각 집단에서 변수 (연령, 교육 수준, 가계 소득, 배우자 유무, 음주, 운동, 체질량지수, 현병력, 직업, 상담 횟수, 하루 흡연량, 흡연 시작 연령)들의 비율을 알아보았고, 시점별 금연 성공률을 집계하였다. 변수와 금연 성공의 관계를 알아보기 위해 각 시점에서 다중 로지스틱 회귀 분석을 실시하였다. 또한 시간 경과에 따른 금연 성공률에 영향을 미치는 변수를 평가하기 위해 Cox 생존 분석을 시행하였다.
결과
SWESC가 있는 근로자의 24주 금연 성공률 (90.7%)은 SWE가 없는 근로자 (60.5%)보다 높았다. 캠프 후 6주, 12주, 24주째 금연성공률과 변수에 대하여 다중 로지스틱 회귀 분석을 실시한 결과, 세 시점에서 모두 금연에 유의한 (p < 0.05) 변수는 SWESC로 나타났다. 모든 변수를 조정하여 Cox 비례 위험 생존 분석을 실시한 결과 SWESC의 위험비는 6.17 ( 95% 신뢰구간: 3.08-12.38)였다.
결론
금연 캠프 참가자 중 SWESC가 있는 참가자는 금연 성공률이 유의하게 높았다. 근로자의 건강에 대한 지지적인 직장 환경은 금연 사업 등 보건 사업의 성공에 중요한 요인이 될 것으로 예상된다.

Marginal socio-economic effects of an employer’s efforts to improve the work environment

Background

Workplace health promotion (WHP) strongly requires the employer’s efforts to improve the psychosocial, ergonomic, and physical environments of the workplace. There are many studies discussing the socio-economic advantage of WHP intervention programmes and thus the internal and external factors motivating employers to implement and integrate such programmes. However, the socio-economic impacts of the employer’s multifactorial efforts to improve the work environment need to be adequately assessed.

Methods

Data were collected from Swedish company Sandvik Materials Technology (SMT) through a work environment survey in April 2014. Different regression equations were analysed to assess marginal effects of the employer’s efforts on overall labour effectiveness (OLE), informal work impairments (IWI), lost working hours (LWH), and labour productivity loss (LPL) in terms of money.

Results

The employer’s multifactorial efforts resulted in increasing OLE, decreasing IWI and illness-related LWH, and cost savings in terms of decreasing LPL.

Conclusion

Environmental factors at the workplace are the important determinant factor for OLE, and the latter is where socio-economic impacts of the employer’s efforts primarily manifest.

The rapid aging trend in South Korea will cause a growing shortage of labor and decreasing quality of the labor force. The purpose of this commentary is to recommend a health care strategy to maintain and promote the work ability of employees in an aging Korea. Strategies to promote the work ability require the collaboration of governmental agencies at the central and local levels. First, the common goal should be the reinforcement of follow-up measure in general medical examinations and the promotion of healthy lifestyles for workers. Second, collaborating activities should be performed among the Worker’s Health Center, the Health Promotion Center, and community health centers. In conclusion, health care strategies for ensuring the work ability in an aging Korea require the collaboration of governmental agencies at the central and local levels.

Effectiveness of a Comprehensive Stress Management Program to Reduce Work-Related Stress in a Medium-Sized Enterprise

Objectives

To assess the effectiveness of a comprehensive workplace stress management program consisting of participatory action-oriented training (PAOT) and individual management.

Methods

A comprehensive workplace stress management program was conducted in a medium-sized enterprise. The baseline survey was conducted in September 2011, using the Korean Occupational Stress Scale (KOSS) and Worker’s Stress Response Inventory (WSRI). After implementing both organizational and individual level interventions, the follow up evaluation was conducted in November 2011.

Results

Most of the workers participated in the organizational level PAOT and made Team-based improvement plans. Based on the stress survey, 24 workers were interviewed by a researcher. After the organizational and individual level interventions, there was a reduction of several adverse psychosocial factors and stress responses. In the case of blue-collar workers, psychosocial factors such as the physical environment, job demands, organizational system, lack of rewards, and occupational climate were significantly improved; in the case of white-collar workers, the occupational climate was improved.

Conclusions

In light of these results, we concluded that the comprehensive stress management program was effective in reducing work-related stress in a short-term period. A persistent long-term follow up is necessary to determine whether the observed effects are maintained over time. Both team-based improvement activities and individual interviews have to be sustainable and complementary to each other under the long-term plan.

Comparison of Cardiovascular Disease Status Between Large Scale Industry Office and Self Employed Male Workers

OBJECTIVES
The aim of this study is to compare the cardiovascular disease status between large scale industry office and self employed male workers who have gaps in their intensive health management.
METHODS
The cross sectional study was carried out with subjects composed of 244 shipyard office male workers and 381 self employed male workers, aged 30 to 55 years. They were given a health exam in a general hospital from January 2007 to June 2009. Information was based on data from self-administered questionnaires, physical examination and laboratory results on blood samples. The degree of coronary artery calcification and stenosis was evaluated by a 64 channel multi-detector computed tomography angiography.
RESULTS
The prevalence of hypertension(p<0.01) and diabetes mellitus(p=0.02) and waist circumference(p<0.01) was significantly lower in the shipyard office workers than in self employed workers. Proper drinking(p<0.01) was more commmon in shipyard office workers. The rates of coronary artery calcification and stenosis were estimated to be 18.4% and 11.5% respectively for shipyard office workers and were significantly lower than the 26.5% and 21.5% for self employed workers. After adjustment for age, the probability of coronary artery stenosis in shipyard office workers was lower than in self employed workers (OR=0.56, 95% CI=0.35~0.90).
CONCLUSIONS
The study results verified the benefits of a health promotion and intervention program and the healthy worker effect for cardiovascular diseases. More specifically, it verified the benefits regarding coronary artery stenosis in large scale industry office workers. This result should encourage the establishment of health promotion programs and the study of occupational epidemiology.

Predictors of Metabolic Syndrome Among Shipyard Workers and its Prevalence

OBJECTIVES
The purpose of this study was to evaluate the predictors of metabolic syndrome in shipyard workers in the Southwestern part of South Korea and determine its prevalence.
METHODS
The study subjects were composed of 1,198 medical check-up examinees, age 37 to 58 years who were examined in a general hospital from July, 2007 to August, 2008. Information about age, smoking, drinking and exercise was based on data from self-administered questionnaires. We collected job factors, education level and marital status from company personnel data and the indices of metabolic syndrome from the medical check-up.
RESULTS
The prevalence of metabolic syndrome among the 1,198 shipyard workers was 15.0%. The prevalence of the components of metabolic syndrome was as follows: 26.9% for central obesity, 31.7% for hypertension, 32.2% for hypertriglyceridemia, 19.2% for low HDL-cholesterolemia and 5.2% for hyperglycemia. Adjusted odds ratio for metabolic syndrome was significantly decreased in productive workers (OR: 0.500, 95% CI: 0.327-0.764). Furthermore, factors such as age, smoking, drinking, exercise, work duration and job type were significantly associated with metabolic syndrome.
CONCLUSIONS
Our findings suggest that even though shipyard workers work for the same employer, the prevalence of metabolic syndrome varies to job type, shift work and work duration. Therefore, general metabolic syndrome management programs for shipyard workers should be designed and tailor-made for specified job characteristics to prevent cardiovascular disease more effectively. Also, thorough control and management of the tailor made metabolic syndrome management programs is needed in the early stages of metabolic syndrome for good health outcomes.

A Workplace Cardiovascular Health Promotion Program and its Short-term Health Effects

OBJECTIVES
Cardiovascular disease is the leading cause of death and a major source of workers' compensation claims in Korea. Since 2000 the Korea Occupational Safety and Health Agency (KOSHA), working through local occupational health institutions, has supported cardiovascular disease prevention programs at a number of companies in Korea. The purpose of this study was to assess the short-term effects of this effort.
METHODS
A total of 11,077 workers at risk were enrolled in the workplace cardiovascular disease prevention program and 5,902 workers (53.3%) completed the 1-year course during 2007. The program consisted of a medical checkup and health counseling for the workers by occupational health nurses. The guidelines for this prevention program were adopted from KOSHA Code H-11-2004. To determine the program's effectiveness, the workers' risks for cardiovascular disease were assessed before and one year after completion of the program.
RESULTS
The intervention led to significant reductions in the mean systolic and diastolic blood pressures of 4.9 mmHg and 3.1 mmHg, respectively. Mean total cholesterol and BMI were also reduced significantly by 8.4 g/dl and 0.1 kg/m2. The rate of smoking was decreased by 6.0% and the percentage of workers engaging in regular exercise was increased by 23.1%. Of the 3,530 workers with the low risk and above, the overall cardiovascular risk was improved in 1,734 (49.1%) of them.
CONCLUSIONS
The cardiovascular disease prevention program supported by the Korea Occupational Safety and Health Agency reduces cardiovascular diseases risks among workers and may improve the health status of workers in Korea.

The Relation between Employees' Lifestyle and Their Health Status in an Electronics Research and Development Company

BACKGROUND
An unhealthy lifestyle is the most important cause of lifestyle-induced illness (diabetes, hypertension, liver enzyme abnormality, dyslipidemia etc.). This study was carried out to evaluate that relation between employees' lifestyle and their health status in a Korean electronic research and development company.
METHODS
We conducted a descriptive cross-sectional survey. 3,041 consecutive workers in a Korean R&D company were enrolled in this study. A checkup list for the health examination (from the National Health Insurance Coperation), which contained the information on the lifestyle, and the results of the health examination, which contained information on the body mass index (BMI), systolic blood pressure (SBP), and diastolic blood pressure (DBP), and liver function tests (LFT, AST, ALT, gamma-GTP) and cholesterol profiles (total cholesterol, TG, HDL, LDL) were done for all the subjects. We evaluated the health profile as normal or abnormal. Statistical analysis of the health profiles and the lifestyle of the employees' was performing by using multiple logistic regression analysis.
RESULTS
The prevalence of 'excessive alcohol drinking' was 52.9% (n=1,610), that for 'lack of exercise' was 86.2% (n=2,662), that for 'meat eating' 12.2% (n=370) and that for 'current smoker' was 31.3% (n=952). According to results of health examination, the prevalence of obesity was 27.1% (n=824), hypertension was 7.1% (n=215), diabetes was 1.0% (n=30), abnormal LFT was 15.4% (n=469) and dyslipidemia was 18.7% (n=569). On multiple logistic regression analysis of the health profile and lifestyle, 'meat eating' was related to obesity, hypertension, abnormal LFT (ALT, gamma-GTP), dyslipidemia (LDL). 'Excessive alcohol drinking' was related to obesity, hypertension, abnormal LFT (gamma-GTP). 'Lack of exercise' was related to dyslipidemia (HDL). 'Current smoker' was related to obesity, diabetes, abnormal LFT (ALT, gamma-GTP), and dyslipidemia (TG, HDL) with statistically significance.
CONCLUSIONS
For workers in the R&D company, an unhealthy lifestyle (such as diet, alcohol drinking, lack of exercise and smoking) were related to obesity, hypertension, diabetes, abnormal LFT and dyslipidemia. Therefore health promotion is necessary for the young employees of an Korean R&D company.

OBJECTIVES
To evaluate the effectiveness of an anti-smoking readership training program on the knowledge, attitude and practice in health managers and supervisors in the workplace.
METHODS
We surveyed 482 health managers and supervisors in the workplace from April 2005 to September 2005. The questionnaire included whether or not a smoking cessation program was conducted in their workplace. We assessed before and after educations program about knowledge, attitude and practice for smoking cessation. The 10-hour education program included the benefit of quit smoking, the law for smoking restriction, cognitive therapy, stress management, nicotine addiction, replacement therapy, case study about workplace for anti-smoking policy and program.
RESULTS
For the change in the mean scores of all items in knowledge, attitude and practice were significantly increased after the intervention.
CONCLUSION
Even a short, 10-hour intervention course of anti-smoking leadership training program was effective for improving the knowledge, attitude, practice to quit smoking activity in workplace. Nevertheless, the program effectiveness would be increased by including a re-training course and website support.

Factors Affecting the Health Promotion Activities of Workers

OBJECTIVE: The aim of this paper was to develop efficient and practical health promotion strategies in the workplace by investigating both the present status of health promotion and the barriers to its practice.
METHODS
A questionnaire was administered to 20% of industrial workers in the workplaces which receives the health care agency enterprise or are supported by a grant from the national treasury, K University Hospital in Gyeong-Gi province, and C University Hospital in Seoul.
RESULTS
According to the questionnaire survey, 51.3% workers were practicing health promotion activities and smoking cessation was the most common method among them. Regarding the factors that prompt workers to commence health promotion activities, TV or the mass media had a 33.4% effect, followed by a 22.1% effect for an abnormal finding at regular health screening. The most necessary health promotion activity in the workplace was exercise (41.9%), followed by stress management. When asked about their expectations of the health promotion program, 84.8% of the respondents answered that they expected it to be significantly or somewhat effective. Factors affecting the health promotion activities of workers were worker's health status and job control.
CONCLUSIONS
This study demonstrated that workers evaluate health promotion activities positively and that some workers are unable to participate in these activities despite their willingness to do so due to the barrier of circumstances. To encourage worker's participation, the government and employers should remove barriers.

Effectiveness of Tailored Health Promotion Program for Reducing Cardiovascular Risk Factors in Subway Workers

OBJECTIVES
This study explores the effects of a tailored health promotion program, in order to reduce cardiovascular risk, in areas which include hypertension, dyslipidemia, abnormal blood glucose and body mass index.
METHODS
In this paper, health risk appraisal relating to lifestyle was surveyed and physical fitness, blood pressure and blood lipid profile and glucose were checked. Then, a 12 week tailored health promotion program with 40 subway workers of Seoul was conducted. The Tailored health promotion program included a smoking cessation program, drinking restriction, diet, exercise prescription and a stress management program.
RESULTS
The number of subjects engaging in regular exercise increased from 42.5% to 100%, and the number of smokers reduced from 72.5% to 60%, and 12.5% of contemplate stage was shift to action stage. The mean level of drinking per day, significantly decreased from 34.9 g to 24.2. The body mass index, systolic blood pressure, total cholesterol, triglyceride, and Fasting blood glucose significantly decreased after the intervention program. In addition the tailored health promotion program resulted in a significant reduction in two factors for the cardiovascular high risk group, hypertension above 140 mmHg in systolic blood pressure and dyslipidemia above 200 mg/dl in triglyceride.
CONCLUSION
In this paper, it is demonstrated that the tailored health promotion program,, including the smoking cessation program, drinking restriction, diet, exercise prescription and stress management program, resulted in effective reduction in cardiovascular risk factor. Therefore associated workplace tailored health promotion programs relating to specific individual risk factor, should be developed and implemented.

OBJECTIVES
To determine the health behavior practices and needs for health promotion in shift work pattern in subway workers.
METHODS
Among 789 male subway workers divided into four groups, 191 day time workers, 187 station service men (3 groups, 2 turns), 204 rolling stock inspectors (3 groups, 2 turns) and 207 train operators (8 groups, 5 turns), we surveyed the health behavior and needs for health promotion by questionnaire.
RESULTS
For obesity, blood pressure and total cholesterol, day time workers had the highest levels, while train operators had higher diseases of the liver and stomach than others. The station service men had the highest smoking rate and daily smoking amount. More than 80% of the respondents consumed alcohol, and the drinking amount was highest among the day time workers and the 8/5 shift train operators. Eighty-two percent of the station service men had irregular eating habits and 17.7% did not have any breakfast. In health promotion activities, exercise and restriction of one's favorite(smoking or drinking) were most preferred in the four groups of respondents. The priority of needs and participation for health promotion program was exercise programs, followed by stress management, cancer prevention program, weight control program, anti-smoking campaign, high blood pressure control program, and abstinence/moderation in drinking.
CONCLUSION
As the health behavior practices differed relative to shift work patterns, it was suggested that a comprehensive health promotion program according to shift work pattern should be planned and implemented among subway workers.

The Relationship between Health Value Cognition, Health Promotion Behavior and Health Examination Results Among Transit Corporation's Workers

OBJECTIVES
This study was performed to compare health value cognition and health promotion behavior compliance between healthy group and not being healthy group, and to evaluate the difference of health promotion behaviors compliance between high group and low group in health value cognition.
METHODS
The subjects of this study were 565 workers, 233 healthy workers, 172 observation cases and 160 disease cases, selected from 4,919 transit coporations workers.
RESULTS
In health promoting behavior compliance, not being healthy group showed lower level than healthy group in exercise and showed higher level than healthy group in interpersonal support significantly. A worker who replied on health as the most value in life was consisted in 37.3% of healthy group and 36. 1% of not being healthy group, Health value cognition according to general characteristics showed no significant difference except only variable of age in healthy group. Health promoting behavior compliance according to cognition of health value showed in healthy group that a high level group of health value cognition was more significant difference than a low level group in self actualization and health responsibility of health promoting behavior and didn't show difference significantly in not being healthy group.
CONCLUSIONS
In conclusion, the relationship between health value cognition, health promotion behavior compliance and periodic health examination results showed weakly. Therefore, in order to develop and apply spontaneous health promotion program, it was considered that should emphasize compliance than cognition.

OBJECTIVES
In order to evaluate the stage of behavior change for health promotion strategy in workplace METHODS: Questionnaire survey was carried out for 309 workers who employed in H Electronics company in Kyongin area from September to October 1998.
RESULTS
According to need assessment of four step such as precontemplation stage, contemplation stage, action stage, maintenance stage which were to change of behavior for health promotion, the percent for quitting smoking were observed 48.8%, 43.1%, 8.1%. 0 %, and for the restriction of drinking were 29.8%, 10.6%, 9.3%, 5.3%; for fitness 11.1%, 31.0%, 37.3%, 20.6% in male 26.6%, 27.8%, 26.6%, 19.0% in female, for regular diet 26.1%, 19.8%, 38.7%, 15.3% in male, 27.8%, 22.2%, 40.0%, 10.0 % in female and for stress management 5.8%, 10.6%, 8.7%, 75.0% in male, 13.6%. 22.7%, 19.7%, 44.0% in female. In heavy smoker (p=0.017) and heavy drinker, (p=0.021) the percent of precontemplation stage was significantly higher than in lower drinker and light smoker.
CONCLUSIONS
This study suggested that company support for health promotion in workplace was weak and workers of high risk group for smoking and drinking were in precontemplation stage. So before conducting the health promotion program, health risk appraisal and need assessment for stage of behavior change should be taken for the efficient heath promotion strategy.

Before and after conducting the health promotion program by group occupational health service during 5 years, we surveyed knowledge, attitude and practice about general health(30 items) and occupational health (30 items) among 25 small and medium scale industry workers (before : 355 workers, after : 279 workers) for evaluation of effectiveness. The scores of knowledge for smoking, drinking, cholesterol, mental stress, management of hypertension, and risks in younger or women workers were significantly increased after conducting group occupational health service. The scorers of attitude for controlling of fatty food consumption and cleaning workplace and bathing for health and that of practice for taking the periodic health examination and choking blood pressure were significantly increased but most of items in attitude and practice didn't be changed after services. According to sex, the scorers of men were significantly increased in knowledge of general health, but there were not significant differences in age, job status and work duration group. In conclusion, through the group occupational health service during 5 years, only the scores of knowledge in general health were increased. It suggested that the effective program which can change workers' attitude and practice for health promotion, should be developed and conducted in small and medium scale industry workers.

This study was conducted to identify the demand of work-site health promotion program of employees. A self-administered questionnaire method was uses to collect data from 492 employees from 40 work-sites in Taegu City and Kyungpook Provincial area. The work-site health screening programs conducted for several years were placed low value by employees. The out-door exercise was the most popular heath promotion activity implemented at work-site as a whole. The employees expressed that periodic health check-up, the health counseling, indoor exercise would be implemented as a health promotion program in the future. The male employees regarded that the periodic health check-up program should be established preferentially at work-site. However, the female employees gave priority to fitness program. The health related behavioral patterns slightly influenced the preference of health promotion program. The health screening program for employees should be improved and the health education of nutrition and diet for female employees should be implemented.
