BACKGROUND We performed a follow-up study of an acute organotin poisoning case on the neurological sequelae that remained after 3 years since diagnosis. CASE In the previous study, a 43-year-old male was reported for acute organotin poisoning. After 3 years, the neurological sequelae were studied using neurological examination, cognitive tests, neurobehavioral tests (by CAYSYS 2000(TM)), brain magnetic resonance imaging (MRI), and 18F-fluorodeoxyglucose (18F-FDG) PET/CT. Upon neurological examination and other tests, orientation was improved compared to that 3 years before, but cerebellar ataxia, dysmetria and dysdiadochokinesia remained the same, with little change. Brain MRI showed cerebellar atrophy. 18F-FDG PET/CT revealed mildly decreased metabolic activity in the pons and in both cerebellar hemispheres. DISCUSSION This is the first brain MRI study on cerebellar atrophy caused by organotin poisoning. Cerebellar ataxia was confirmed by decreased metabolic activity in 18F-FDG PET/CT.

BACKGROUND Ethyl cyanoacrylate is used as a component of adhesives used in the industrial, domestic, and medical fields. It is known to cause mucosal irritation, contact dermatitis and respiratory discomfort. However, reports of combined occupational asthma and rhinitis are rare, especially in Korea. We report a case of occupational asthma and rhinitis suspected to be caused by exposure to ethyl cyanoacrylate. CASE REPORT: A 36-year-old woman presented with rhinorrhea, cough, dyspnea and chest discomfort after exposure to ethyl cyanoacrylate adhesive at an assembly area of a plastic products manufacturing company. Nasal endoscopy revealed watery rhinorrhea and hypertrophy of nasal turbinates, which were compatible with rhinitis. The methacholine provocation test showed a positive result. Daily variability of peak expiratory flow increased on work days, but decreased during non-work days. Based on these findings, we diagnosed this case as a combination of occupational asthma and rhinitis suspected to be caused by exposure to ethyl cyanoacrylate. CONCLUSION There needs to be increased awareness regarding occupational asth-ma and rhinitis caused by exposure to ethyl cyanoacrylate.
BACKGROUND Epoxy resin compounds are one of the common causes of occupational allergic contact dermatitis. In Korea, most cases of allergic contact dermatitis from epoxy resin compounds have been caused by the epoxy resin itself. We report a rare case of allergic contact dermatitis which was caused by epichlorohydrin, an ingredient of epoxy resin and 2,4,6-tris-(dimethylaminomethyl)phenol (tris-DMP), a kind of hardeners. CASE REPORT: A 43-year-old man, who had worked at the epoxy resin glue manufacturing factory since 1999, presented with mild and intermittent erythematous papules and rashes on his face, neck, trunk, and both arms. He was dealing with epoxy resin, epichlorohydrin, bisphenol A and hardeners. After a new hardener was added in August 2008, his skin lesions worsened from what he had experienced in the past. A skin patch test was performed to identify the causative chemicals of the skin lesion. Epichlorohydrin and tris-DMP elicited positive reactions after 48 hours and increased after 96 hours. CONCLUSION This case confirmed occupational allergic contact dermatitis caused by epichlorohydrin and tris-DMP, an ingredient of epoxy resin and a hardener, respectively.

OBJECTIVES This study was performed to investigate the factors related to HRQOL, and a correlation between occupational stress and HRQOL among municipal fire officers. METHODS In November 2005, 341 male municipal fire officers submitted to the following tests: A general questionnaire, a medical questionnaire, the Korean Occupational Stress Scale Short Form(KOSSSF), and the SmithKline Beecham quality of life scale (SBQOL). RESULTS Back pain (OR=3.54, 95% C.I.=1.81-6.93) and occupational stress (OR=6.33, 95% C.I.=3.12-12.83) were significantly related to poor HRQOL scores among municipal fire officers, and there is negative relationship between KOSS-SF score and SBQOL score. Logistic regression analysis indicated that the medium occupational stress group (OR=2.21, 95% C.I.=1.04-4.71) and the high occupational stress group (OR=6.33, 95% C.I.=3.12-12.83) were more significantly associated with poor HRQOL than low occupational stress group. CONCLUSIONS Among municipal fire officers, back pain and occupational stress were found to be related to their HRQOL. Based on this study, we suggest that the management of back pain and occupational stress are instrumental to improving the HRQOL among municipal fire officers.

OBJECTIVE: It is well established that manganese neurotoxicity is associated with clinical symptoms similar to those of idiopathic Parkinson's disease. Recent research has shown that the exposure to manganese (MnCl2) leads to induction of iNOS in BV2 microglial cells via iNOS transcriptional up-regulation and activation of both MAPKs and PI3K/Akt signaling pathways. Here, we further investigated the effect and the action mechanism of MnCl2 on iNOS expression in C6 glioma cells. METHODS Western blot analyses demonstrated that treatment with MnCl2 at 250 micronmeter was sufficient to induce iNOS at both the protein and mRNA levels in C6 cells. RESULTS These studies demonstrated that the induction of iNOS protein and mRNA was visible after 4h- and 2 h-treatment with MnCl2, respectively. MnCl2 treatment led to strong phosphorylation of JNKs and ERKs, members of MAP kinases (MAPKs), and Akt, a PI3-kinase (PI3K) downstream effector, in C6 cells. MnCl2 treatment had no effect on I kappa B-alpha in C6 cells. Notably, pretreatment with LY294002 (a PI3K inhibitor), which inhibited phosphorylation of Akt by MnCl2, caused strong suppression of MnCl2- induced iNOS protein and mRNA expression in C6 cells. Moreover, pretreatment with SP600125 (an inhibitor of JNKs) and PD98050 (an inhibitor of ERKs), which respectively interfered with MnCl2-mediated phosphorylation of JNKs and ERKs, led to the partial suppression of MnCl2-induced iNOS protein. Interestingly, pretreatment with LY294002 inhibited phosphorylation of not only Akt, but also ERKs and JNKs, in response to MnCl2. Moreover, there was an effective suppression of MnCl2-mediated phosphorylation of AKT by SP600125. CONCLUSION These results collectively suggest that MnCl2 induces iNOS expression in C6 glioma cells via activation of PI3K/Akt and JNK-ERK MAPK signaling proteins, whose activations seem to be mutually interconnected in response to MnCl2.
OBJECTIVES To investigate the effect of the occupational exposure to organic solvent on the olfactory function of shipyard painters; we measured the prevalence of olfactory function impairment between the exposure group and the controls by using the Korean Version of the Sniffin'Stick (KVSS) test through smell, and by analyzing factors related to olfactory alteration. METHODS The olfactory function of 245 painters exposed to organic solvents was compared to that of 122 unexposed controls. A total of 367 shipyard subjects underwent questionnaire study and the olfactory function test was performed by KVSS test I from April to August 2007. Suspected subjects of abnormal findings were referred to an otorhinolaryngologist. In addition, the Biologic Exposure Index (BEI) and industrial hygiene laboratory data were collected as well as analyzed by job title in the exposure group. RESULTS The difference in the result of KVSS test I was significantly different from the exposure group and the controls (P<0.05). For the exposure group, reduced olfaction and exposure group were correlated with statistical significance when compared to the controls (P<0.01). Adjusting for work duration, smoking, rhinitis or sinusitis history, exposure of the organic solvent (OR 1.76, 95% CI=1.05-2.96) had a statistically meaningful relationship with decreased olfaction. The data on BEI for the exposure group were collected; the difference in methylhippuric acid was significant between spray workers and others, but did not correlate well with decreased olfaction. CONCLUSION Occupational exposure to organic solvents in shipyard painters may affect the olfactory function. Therefore, shipyard painters should be monitored on a periodic bases by the quantitative test such as KVSS test. There is no other quantitative testing method or standardized procedure that can objectively diagnose the neurotoxic effect, further enhanced quality research in this field is necessary.

BACKGROUND The purpose of the study was to estimate the mental health problems and other related factors in residents that experienced the Samsung-Hebei spirits oil spill by surveying PTSD symptoms. METHOD Trained interviewers performed direct interviews of the residents approximately 70 days after the oil spill. We investigated PTSD symptoms through PTSD Symptom Scale Interview Version (PSS-I). To determine those factors related to PTSD symptoms, the following factors were determined as part of the survey: gender, age, occupation, duration of cleanup activity, monthly income and amount of debt. Logistic regression analysis was used to analyze all factors. RESULTS Of the 318 residents investigated, 56.6% of the subjects had PTSD symptoms related to the spirits oil spill (about 70 days post spill). With regard to occupation, those subjects answering fishery (POR:3.05) and commerce (POR:4.24) as their occupations experienced higher PTSD symptoms than residents answering farming as their occupation. Residents who answered that they had debt over 10 million KRW (POR:2.61) were more vulnerable to PTSD symptoms compared to residents without debt; residents with acute physical symptoms were vulnerable (POR:5.11) to PTSD symptom compared to residents without acute physical symptoms. The results of multiple logistic analysis, including the cleanup activity, age, gender, occupation, acute physical symptoms and amount of debt in the model suggest that only cleanup activity increased PTSD symptoms. The subjects who had engaged in cleanup activities for longer periods of time had more PTSD symptoms and an additional dose-response relationship. CONCLUSION Many residents in Samsung-Hebei spirits oil spill area had PTSD symptoms. This suggests that there were serious mental health problems among the residents, who might require specific social supports and psychiatric interventions as a result of the oil spill.

OBJECTIVE: To investigate the effects of cardiovascular risk factors on hearing loss by vascular damage, such as in atherosclerosis. METHOD This study was conducted as a part of a study for the health effects of air-craft noise from 13 July 2005 to 13 January 2006. In this study 810 residents were selected from 13 villages in Pyeongteck City. Participants in this study were stratified by gender. Individuals, who had conductive hearing loss, were excluded form this investigation. Cardiovascular risk factors were divided into medical risk factors (mean arterial pressure (MAP), blood glucose, triglyceride) and behavioral risk factors (smoking, alcohol). The degree of atherosclerosis was determined by aPWV. Pure tone air conduction hearing thresholds were obtained at frequencies of 1000 and 4000 Hz. As for statistic data analysis, multiple linear regression models were used to evaluate the relationship between factors on hearing loss and aortic pulse wave velocity. Structural Equation Modeling was used to analyze the relationship between factors. RESULTS According to multiple linear regression models among males, age, aircraft noise, aPWV were associated with hearing loss. In females, age and aircraft noise were associated with hearing loss. Thus cardiovascular risk factors were not directly involved in hearing loss. However, cardiovascular risk factors such as alcohol, MAP in males and MAP, blood glucose in females were statistically significant (p<0.05) on aPWV according to multiple linear regression modeling. According to analysis of structural equation modeling, an indirect relationship was observed between cardiovascular risk factors and hearing loss by atherosclerosis, which was measured by aPWV. In males, medical risk factors were significantly associated with atherosclerosis (beta+/-SE: 0.07+/-0.03, p=0.03). Atherosclerosis was also associated with hearing loss (beta+/-SE: 2.01+/-0.89, p=0.03). In females, medial risk factors was significantly associated with atherosclerosis (beta+/-SE: 0.08+/-0.02, p=0.00), but atherosclerosis was not significantly associated with hearing loss. CONCLUSION Although noise exposure is a known major threat to hearing loss, understanding of cardiovascular risk factors associated with hearing loss also take a significant role in preventing hearing loss. Therefore, scrutinization of the etiologic factors for hearing disability may introduce a detailed strategy to abate the prevalence of hearing loss.
OBJECTIVE: To evaluate the changes in respiratory function associated with daily changes in particulate pollution. METHODS A total of 105 healthy schoolchildren of fourth and fifth grade elementary school age performed peak expiratory flow rates (PEFR) during 2 periods (May 14~June 15, Oct 22~Nov 23). We monitored concomitant ambient particulate matter (PM) levels during the periods. RESULTS An elevation PM(10) level of 1 microgram/m) led to decreases in PEFR of 0.20 L/min. A 1 microgram/m3 elevation of PM(2.5) led to a decrease of 0.36 L/min in PEFR. In addition, 1 day lag was associated with PM(10) levels and PEFR changes, and PM(2.5) showed the same result. The metal levels determined at PM(10) and PM(2.5) were found to be associated with decreased respiratory function. CONCLUSIONS An association between respiratory function and particulate pollution was observed when particulate pollutant levels were lower than the yearly average National Ambient Air Quality Standard in the Korea(50 microgram/m3).

OBJECTIVES The purpose of this study was to evaluate the predictors of metabolic syndrome in shipyard workers in the Southwestern part of South Korea and determine its prevalence. METHODS The study subjects were composed of 1,198 medical check-up examinees, age 37 to 58 years who were examined in a general hospital from July, 2007 to August, 2008. Information about age, smoking, drinking and exercise was based on data from self-administered questionnaires. We collected job factors, education level and marital status from company personnel data and the indices of metabolic syndrome from the medical check-up. RESULTS The prevalence of metabolic syndrome among the 1,198 shipyard workers was 15.0%. The prevalence of the components of metabolic syndrome was as follows: 26.9% for central obesity, 31.7% for hypertension, 32.2% for hypertriglyceridemia, 19.2% for low HDL-cholesterolemia and 5.2% for hyperglycemia. Adjusted odds ratio for metabolic syndrome was significantly decreased in productive workers (OR: 0.500, 95% CI: 0.327-0.764). Furthermore, factors such as age, smoking, drinking, exercise, work duration and job type were significantly associated with metabolic syndrome. CONCLUSIONS Our findings suggest that even though shipyard workers work for the same employer, the prevalence of metabolic syndrome varies to job type, shift work and work duration. Therefore, general metabolic syndrome management programs for shipyard workers should be designed and tailor-made for specified job characteristics to prevent cardiovascular disease more effectively. Also, thorough control and management of the tailor made metabolic syndrome management programs is needed in the early stages of metabolic syndrome for good health outcomes.
