Effect of maternal job strain during pregnancy on infant neurodevelopment by gender at 6 and 12 months: Mothers and Children’s Environmental Health (MOCEH) study

Objectives

Limited evidence is available regarding the association between prenatal job strain and infant neurodevelopment. Most studies used stress indicators other than job strain to explain the relationship between prenatal maternal stress and child development. The objective of this study was to investigate the association between maternal job strain during pregnancy and neurodevelopment in infancy.

Methods

Mothers and Children’s Environmental Health (MOCEH) study, an on-going prospective birth cohort study, has been conducted in South Korea since 2006. Job strain during pregnancy was measured using Korean version of Job Content Questionnaire (JCQ). Infant neurodevelopment was assessed using Korean Bayley Scale of Infant Development II (K-BSID-II) at 6 and 12 months of age. A total of 343 mother-child pairs that completed JCQ and K-BSID-II more than once were included. Mental Developmental Index (MDI) and Psychomotor Developmental Index (PDI) defined in the K-BSID-II were used as outcome variables.

Results

Compared to infants from mothers with low job strain, significant (p < 0.05) decreases in PDI were found in infants from mothers with active and passive job at 6 months of age. After stratification by infant sex, boys in the high strain group had a lower MDI score than boys in the low job strain group at 12 months. On the other hand, girls in the high strain and active groups had higher MDI scores than girls in the low job strain group at 12 months. PDI at 12 months also showed different results by sex. Boys in the high strain and passive job groups had lower PDI scores than boys in the low job strain group. However, such difference was not observed in girls.

Conclusions

The findings of this study suggest that prenatal job strain affects infant neurodevelopment in a gender-dependent manner.

Mercury Concentrations of Maternal and Umbilical Cord Blood in Korean Pregnant Women: Preliminary Study

OBJECTIVES
To evaluate the level of maternal and prenatal mercury exposure and to analyze the related factors.
METHODS
Fifty-nine pregnant women were recruited into this study after obtaining informed consent. Samples were collected at delivery from normal pregnant women who were living in the city of Busan, Korea. Mercury concentrations in maternal and umbilical cord blood samples were measured using a gold-amalgam collection method. The total and methyl mercury levels of 36 of the 59 pregnant women were analyzed after randomization, and the results were compared.
RESULTS
The mean total mercury concentration was 3.16+/-1.21 ppb and 5.43+/-2.22 ppb in maternal and cord blood, respectively. The average, maternal blood mercury level was lower than the prescribed toxic limit for human (WHO, 5 ppb), whereas the cord blood mercury was higher. The mercury exposure level exceeded the WHO recommendation in 5 (8.47%) cases of maternal blood and 29 of (49.15%) cord blood. There was a significant correlation between maternal and cord blood mercury concentrations. Total mercury and methyl mercury concentrations of the 36 random pregnant women were 3.06+/-1.17 ppb, and 2.60+/-1.11 ppb in maternal blood, and 5.20+/-2.36 ppb, and 4.70+/-1.97 ppb in cord blood, respectively. Methyl mercury accounted for 85.0% of the total mercury in maternal blood and 90.4% in cord blood. There was a significant correlation between total and methyl mercury concentrations.
CONCLUSIONS
The study results suggest that mercury concentrations of cord blood may be regarded as indicative of high prenatal mercury exposure. Therefore, further studies are necessary to explain the cause of high mercury concentrations in cord blood, and to examine its relationship with various health indices.

OBJECTIVES
The aim of this study was to examine the association of prenatal environmental exposure and pregnancy outcomes.
METHODS
Self-administered questionnaire survey was conducted for pregnant women with gestational age of 16-18 weeks, who visited the outpatient department of Dankook University Hospital from May 1995 to June 1999. Of all 1170 women, 762 women answered the questionnaires and the response rate was 65 %. Among them, 471 cases which has delivered at our hospital were included in this study.
RESULTS
Comparing the environmental risk factors and the pregnancy outcomes, there was an association between delivery of small for gestational age infants and the ingestion of herb medicine and caffeine-containing nutritional beverages. Pregnant women with occupation were more likely exposed to environmental tobacco smoking, to ingestion of nutritional drinks and green teas, to use of video display terminals and to diagnostic radiation.
CONCLUSIONS
We could get precise information from prospectively conducted questionnaire study. It is necessary to extend the study population to verify the influence of prenatal environmental factors on pregnancy outcomes.
