Incidence rates of injury, musculoskeletal, skin, pulmonary and chronic diseases among construction workers by classification of occupations in South Korea: a 1,027 subject-based cohort of the Korean Construction Worker’s Cohort (KCWC)

Background

The objective of this study is to investigate the differences in incidence rates of targeted diseases by classification of occupations among construction workers in Korea.

Methods

In a subject-based cohort of the Korean Construction Worker’s Cohort, we surveyed a total of 1,027 construction workers. As occupational exposure, the classification of occupations was developed using two axes: construction business and job type. To analyze disease incidence, we linked survey data with National Health Insurance Service data. Eleven target disease categories with high prevalence or estimated work-relatedness among construction workers were evaluated in our study. The average incidence rates were calculated as cases per 1,000 person-years (PY).

Results

Injury, poisoning, and certain other consequences of external causes had the highest incidence rate of 344.08 per 1,000 PY, followed by disease of the musculoskeletal system and connective tissue for 208.64 and diseases of the skin and subcutaneous tissue for 197.87 in our cohort. We especially found that chronic obstructive pulmonary disease was more common in construction painters, civil engineering welders, and civil engineering frame mold carpenters, asthma in construction painters, landscape, and construction water proofers, interstitial lung diseases in construction water proofers.

Conclusions

This is the first study to systematically classify complex construction occupations in order to analyze occupational diseases in Korean construction workers. There were differences in disease incidences among construction workers based on the classification of occupations. It is necessary to develop customized occupational safety and health policies for high-risk occupations for each disease in the construction industry.

한국 건설업 종사자의 손상, 근골격계 질환, 피부, 호흡기 및 만성질환의 건설 직종에 따른 발생률: 한국 건설업 종사자 코호트(KCWC) 의 1027명 대상자 기반 세부 코호트 분석
목적
본 연구의 목적은 한국 건설업 종사자에 있어 주요 질환의 건설 직종별 발생률 차이를 알아보는 것이다.
방법
한국 건설업 종사자 코호트(KCWC)의 대상자 기반 세부 코호트로 총 1,027명의 건설업 종사자가 1:1면접설문조사를 통해 입적되었다. 직업적 노출요인으로 직종은 크게 건설산업환경과 직무 두 가지 축으로 분류되었다: 직업적 노출(설문조사) 자료와 국민건강보험공단 청구 맞춤형 자료를 연계하여 질환의 평균 발생률을 분석하였다. 주요 질환은 건설업 종사자에서 유병률이 높거나 업무 관련성이 있을 것으로 추정되는 11개의 대상 질환을 평가하였다. 질환별 평균 발생률은 1,000인년당 발병 사례수(명)으로 계산하였다.
결과
손상, 중독 및 외인에 의한 특정 기타 결과가 1,000인년당 344.08명으로 가장 높은 발생률을 보였고, 그 다음으로 근골격계 및 결체조직의 질환이 1,000인년당 208.64명, 피부 및 피하조직 질환이 1,000인녕당 197.87명 순으로 높았다. 특히, 만성 폐쇄성 폐질환이 건축 도장공, 토목 용접공, 토목 형틀목공에서 타 직종에 비해 상대적으로 높은 발생률을 보였고, 천식은 건축 도장공, 조경, 건축 방수공에서, 간질성 폐질환은 건축 방수공에서 상대적으로 높은 발생률을 보였다.
결론
본 연구는 한국 건설업 종사자의 주요 직업성 질환 및 호발 질환 등을 복잡한 건설산업의 직종을 체계적으로 분류하여 분석한 최초의 연구로 연구진들은 건설업 종사자의 질환별 발생률에 있어 직종별 차이를 확인하였다. 건설 산업에서 각 질환별 고위험 직종별로 맞춤형 산업안전보건정책 개발이 필요하다.

Epidemiological Characteristics and Scale for Needlestick Injury in Some University Hospital Workers

OBJECTIVES
Based on multicenter research among tertiary hospitals in Korea, it was our intention to effectively contribute to the prevention of needlestick injuries by understanding the current state of the incidence rate of needlestick injuries and the related epidemiology with respect to medical institution and worker's characteristics.
METHODS
Ten hospitals were selected, with the research performed from July to September 2007. Data on cases of needlestick injuries were collected by hospital infection-controlling centers using a specifically developed website and a partially modified self-writing questionnaire based on the Exposure Prevention Information Network (EPINet).
RESULTS
During the study period, a total of 327 needlestick injury cases were reported, with an incidence of 10.56 cases per 100 patient beds per year. The existence of source of infection could not be confirmed in one third of the needlestick injuries. There were 4.07 incidences per 100 employees per year. The job with the highest incidence was medical doctor interns (18.66 cases), followed by clinical laboratory workers (7.12 cases) and registered nurses (6.66 cases).
CONCLUSIONS
There was no difference in number of incidences according to the number of beds. The highest number of cases occurred in the registered nurse-group, but the incidence rate per healthcare worker was the highest in the medical doctor intern-group. Therefore, it is important to conduct research to identify the risk of the incidence with respect to different risk-groups. As a result of this research, it is suggest that a protective policy is required relating to needlestick injuries, regardless of the number of beds and potential cause of infection.

Incidence of Dimethylacetamide Induced Toxic Liver Injury among Workers in a Synthetic Fiber Manufacturing Company

OBJECTIVES
The main goal of this study was to find the incidence of the dimethylacetamide (DMAc) induced toxic liver injury among workers who were exposed to DMAc for about 5 years in a synthetic fiber factory.
METHODS
In our investigation, total 1,021 workers had been exposed to DMAc from 1st February, 2000 to 30th June, 2004. Among them, 24 workers who had initial abnormal hepatic enzyme level or was viral hepatitis B carrier were excluded. Finally, 997 DMAc exposed workers were followed up for their incidence of toxic liver injury. To find out DMAc induced toxic liver injury case, we carried out regular examinations including liver enzyme tests such as AST and ALT.
RESULTS
We followed up 997 workers and among them 72 cases coincided with our case definition. Incidence density method was 6.05 per 100 person-year. and, a cumulative incidence by life table method was 0.0965 within a year. The incidence of the DMAc-induced toxic liver injury peaked at 4 to 5 in weeks after employment. There was no toxic liver injury case within second week after exposure to DMAc. Five new cases developed in 2th to 3th week, 25 cases in 4th to 5th week, 14 cases in 6th to 7th week, 12 cases in 8th to 9th week, 5 cases in 10th to 11th week, 4 cases in 12th to 13th week, 4 cases in 14th to 15th week, 1 case in 16th to 17th week, 1 case in 30th to 31th week, and there was one case in 48th to 49th week.
CONCLUSIONS
The incidence rate of toxic liver injury was considerably high during the 1st year after a worker is placed. So it is needed to take careful monitoring of hepatic enzyme level for newly exposed worker.

Incidence and related factors of fatty liver among male workers

OBJECTIVE: This study was conducted to investigate the incidence rate of fatty liver and it's related factors based on a three year follow up of multiphasic health screening participants.
METHODS
671 male workers who had participated in both initial and second multiphasic health screening were selected. Initial screening was performed during the year of 1996 to 1997 and second screening was performed during the year of 1999 to 2000. Among them, four hundred and thirty workers who didn't initially have a fatty liver were selected as the final study subjects. They were classified into 2 categories according to the initial and second hepatic ultrasonographic findings; stationary normal(307, 71.4%), and fatty liver incident(123, 28.6%). The incidence rate was calculated using the incidence density method.
RESULTS
The incidence rate of fatty liver among the male workers was 9.73 per 100 person-years. The white collar workers had a higher incidence rate(10.66) than the blue collar workers(8.14). The incidence density ratio of alcohol drinking to fatty liver incidence was 1.41, and the incidence density ratio of low vegetable diet was 1.63. The incidence density ratio of obesity was 1.78 for the 100-109% obesity index group, 2.83 for the 110-119% obesity index group, 4.25 for the >or=120% obesity index group over <100%. Smoking, regular exercise, salty food preference, and taking food supplement had no significant effect on the incidence of fatty liver. The fatty liver incident group had higher initial blood pressure, GPT, gamma GTP, fasting blood sugar, HbA1c, uric acid, total cholesterol and triglyceride levels than the stationary normal group. However, there were no differences in the GOT, HDL-cholesterol or LDL-cholesterol levels. In the multiple logistic regression analysis, obesity index and serum triglyceride were found to be major factors in the incidence of fatty liver after controlling age, alcohol drinking, vegetable diet, daily coffee intake, blood pressure, HbA1c, total cholesterol, and uric acid levels.
CONCLUSION
The incidence rate of fatty liver among the male workers was 9.73 per 100 person-years. The lifestyles which gave rise to an increasing incidence of fatty liver were those involving excessive alcohol drinking, obesity, and low vegetable diet. Moreover, many cardiovascular disorder related health indices such as blood pressure, and the blood sugar, blood lipid and uric acid levels, were related to the incidence of fatty liver.

The Incidence Rate of the Pneumoconiosis among Retired Coal Miners

OBJECTIVES
The incidence rate of pneumoconiosis among retired coal miners was studied at Taebackchungang hospital.
METHODS
Newly developed pneumoconiosis0=1/0) cases were gathered from films of ex-coal miners who were diagnosed as normal at the previous examination after retirement.
RESULTS
228 exminers were confirmed as normal at the previous examination after retirement. 37 ex-miners were diagnosed as pneumoconiosis (equal or greater than 1/0) at the next examination.
CONCLUSIONS
The incidence rate was 29.5/1000 person-year. The incidence was calculated by the examined year. In 1995, 1996, 1997, the incidence rates were 104. 8, 37. 1, 14.1 per 1000 person-year respectively. There was no development of pneumoconiosis among retired coal miners who retired more than 20 years ago or who worked less than 10 years.

Incidence of Abnormal Liver Function and Risk Factors in Male Employees

OBJECTIVES
This study was conducted to estimate the incidence of abnormal liver function and risk factors in male employees of an industry in Ulsan City.
METHODS
Five hundreds and seventy nine male employees were selected as the study cohort and 533(92.1%) of them were followed after one year. The blood sample was collected to test for AST, ALT, gamma-GTP, total-cholesterol, fasting blood sugar and a self-administered questionnaire on life style was done. General characteristics(age, marital status, educational level), job department, exposure status for organic solvents, life style(alcohol, smoking, exercise, diet), past history of liver disease, family history of liver disease, drug intake, HBsAg, blood glucose, total-cholesterol were considered as risk factors. The result of liver function test after 1 year follow-up was treated as dependent variable. The operational definition of abnormal liver function was as follows; those who had abnormal liver functions in the two repeated tests with one month interval.
RESULTS
The annual incidence of abnormal liver function was 9.6 per 100 and age-standardized incidence was 9.5. BMI, alcohol, past history of liver disease, and meat intake were significantly related to the incidence(p<0.05). In multiple logistic regression analysis, BMI(RR=2.70, 95% CI=1.41-5.16) and alcohol(RR=1.98, 95% CI=1.08-3.60) were proved as the significant variables. By stratified analysis considering changing pattern of alcohol and BMI, the relative risk of the BMI normal-normal and alcohol intake high-high group was 2.24(95% CI=1.09-4.62) and that of the BMI obese-obese and alcohol intake high-high group was 5.66(95% CI=2.69-11.88) compared with that of BMI normal-normal and alcohol intake low-low group.
CONCLUSIONS
The age-standardized annual incidence of abnormal liver function was 9.5 per 100 in male employees. Thus, an active effort for reducing alcohol intake and controlling BMI should be done to reduce the incidence.

Incidence and Risk Factors for Occupational Low Back Pain Among Shipyard Workers

OBJECTIVES
This study was conducted to estimate the incidence rate, and to identify the risk factors for the occupational low back pain among shipyard workers.
METHODS
The study subjects consisted of 9,784 workers who were employed in a ship-building industry(excluded workers who had a history of low pack pain before 1995 or did not take periodic health examination in 1995). The cases were 220 people who experienced back pain from January 1, 1996 to December 31, 1998. To assess risk factors for occupational low back pain, Cox propotional hazard model was used.
RESULTS
During the recent three years from 1996 to 1998, the incidence rate per 1,000 persons was 7. 8 in 1996, 8. 8 in 1997, and 3. 1 in 1998. The main causes of work-related low back pain were sprain, strain and disc herniation. Lifting was the most common cause of back pain(35. 3%), and carrying(10.2%) and pulling(8.0%) were followed. In Cox proportional harzard model, independent risk factors for back pain were body mass index(R.R.; 1.54, 959o C.I.: 1.06-2.25), shift work(R.R.; 1.65, 95% C.I.: 1.19-2.28), and lifting heavy material(R.R. ; 3.95, 95% C.I: 2.29-6.82).
CONCLUSIONS
This study suggests that the risk factors of back pain in shipyard workers were body mass index, shift work and lifting.

A Status of the Report for Industrial Injuries and Illnesses at an Automobile Related Plant

The incidence rate of industrial accidents tends to decrease since 1984. It seems to be caused by the active prevention activities for industrial accidents, however, there has been some concern for under-reporting of industrial injuries and illnesses. This study was carried out to assess the status of injury occurrences and industrial accident reports. The author reviewed and analysed the record from an infirmary of an automobile related factory from May 1994 to April 1998. The results obtained were as follows; 1. The number of injuries were 665 spells during four years and incidence rates per year per 100 persons were 39.57(1995), 39.04(1996) and 36.86(1997). The incidence rate of industrial injuries that needed over four days of medical treatment were 4.32 %, 6.51 % and 4.90 %, respectively by the year. 2. The highest incidence rate by age was the 20-29 age group (46.30 %) and by working duration was the under five years group (46.18 %). 3. A total of 665 injuries were occurred by 244 workers. Workers who experienced over five accidents were 41 persons (16.8 %) with 285 injuries and the proportion of total spells was 42.9 %. 4. Workers who experienced over 5 accidents were the younger group, had under five years working duration and worked at the position of frequently using hand tools. 5. Among the 665 spells, the injuries that needed over four days of medical treatment were 93 spells (14.0 %) and the reported proportion as an industrial accident was 8.6 % (eight spells). The results suggest that improvement of the report system is required to enhance the accuracy and reliability of the industrial accident statistics, and analysing all accidents would be necessary to setup the strategy for prevention even if they are minor injuries.

Pneumoconiosis, especially Coal-workers' Pneumoconi6sis(GWP), is the principal occupation-related disease in Korea because of the large number of affected workers. Coal mines and miners have been reduced abruptly during recent 8 years, but coal mining should be kept in Korea. Recently, pneumoconiotic workers are increasing in manufacturing industry. It is necessary to know the characteristics of CWP to prevent the development of CWP and manage employed or retired pneumoconiotic workers. Furthermore, it is also necessary to study CWP to protect workers from pneumoconiosis in manufacturing industry. Of the total of 6,452 workers who were diagnosed as CWP initially during the 20 years from 1973 to 1992, X-ray category was as follows: category 1(35.2%), category 2(23.1%), suspicious (0/1 category, 13.4%), category 3(5.7%), large opacity (3.5%), unknown by. complete classification (19.1%). The patients' cardiopulmonary disability was as follows: no disability 79.3%, slight 14.2%, mild 4.1%, moderate 1.9%, severe 0.5%. The patients' X-ray category and disability were not related with the initially exposed age or job position, but their severity was positively related with the exposed duration that was adjusted by the initially exposed age. Also, the patients' X-ray category and disability had positive relationship each other. The cumulative exposure dose of silica/was more important than that'of respirable dust in the. development of large opacity CWP. The annual incidence density of CWP was 73.2 persons in 1982 and 75.8 persons in 1986per 10,000 person years. Afterthen it has been gradually decreasing and was in the range of 20-30 persons in the period of 1989-1992.
