Assessment of Acquired Color Vision Impairment in Inhalant Abusers

OBJECTIVES
This study was conducted in order to evaluate the effects of inhalants on color vision impairment.
METHODS
The inhalation group consisted of 81 neurotoxic chemical substance abusers at Bugok National Hospital and Institute of Forensic Psychiatry, Ministry of Justice. The control group consisted of 41 employees of Bugok National Hospital. The Lanthony D-15 desaturated hue test was administered to both groups. For the qualitative analysis, we classified the results by the morphological characteristics of color vision impairment. For the quantitative analysis, we calculated the total color distance score (TCDS) and the color confusion index (CCI) and determined that the individual was abnormal if the TCDS was over 65.0 or the CCI was over 1.25.
RESULTS
The TCDS of the inhalation group was 75.58+/-21.07 and the CCI was 1.34+/-0.37 in the right eye, and 75.47+/-23.63 and 1.34+/-0.42, respectively in the left eye. These results are higher than those found for the control group. In regards to the TCDS, 50 (61.8%) of the abusers in the inhalation group had color vision impairment in the right eye and 48 (59.2%) had impairment in the left eye. 11 (26.8%) of the employees in the control group had color vision impairment in the right eye and 13 (31.7%) had impairment in the left eye. In regards to the CCI, 38 (46.9%) of the abusers in the inhalation group had color vision impairment in the right eye and 35 (43.2%) had impairment in the left eye. 4 (9.8%) of the employees in the control group had color vision impairment in the right eye and 7 (17.0%) had impairment in the left eye. The results of multiple logistic regression analysis for acquired color vision impairment showed that the inhalation period (OR=1.20, 95% CI: 1.03~1.40) was the significant variable.
CONCLUSIONS
These findings indicate that the inhalation of neurotoxic substances may impair the color vision, and the period inhaled with neurotoxic substances is significant variable affecting on the acquired color vision impairment.

Acquried Dyschromatopsia in Women Workers in Shoe Manufacturing Who were Exposed to Organic Solvents

OBJECTIVES
This study was undertaken to investigate the relationship between color vision defects and occupational exposure to mixed organic solvents in women workers who were engaged in the shoe manufacturing industry.
METHODS
A total of 173 women workers were involved in this study, 85 of whom were exposed to mixed organic solvents and were defined as the exposed group, and 88 workers comprised the non-exposed control group. The patients were questioned as to their drinking history, the year in which they were exposed and whether they had eye symptoms of blurred vision, eye fatigue, and eye irritation. The exposed workers in the shoe manufacturing factory were engaged in 3 work areas which were pasting, trimming, and cleaning. Their Color vision was assessed using the Hahn's double 15 hue test under standard illumination and their current and cumulative exposure levels were measured.
RESULTS
The prevalence of color vision defect was 21.2% in the exposed group and 8.0% in the control group, and the blue-yellow defect was found to be 5.9 % in the exposed group. The logistic regression for the acquired dyschromatopsia and color confusion index showed that there were no variables that had significant relationships. Eye symptoms were more frequently developed in the exposed group.
CONCLUSIONS
There is a possibility of developing color vision defects when workers are exposed to mixed organic solvents. However, the results of the color confusion index showed that there was no significant relationship to the cumulative exposure level, because of the low exposure level and high occupational turn over rate of the workers. Because the workers in the present study were exposed to low level solvents it will be necessary to study workers exposed to higher levels of organic solvents.

Polyneuropathy by Occupational Exposure to Acrylamide

Authors report a first Korean case of polyneuropathy in an acrylamide producing factory together with an environmental and epidemiologic survey for the ten workers of the same workplace and 46 workers in the production and manufacturing acrylamide in Ulsan, Korea. The case is 30 years old man who worked in the prcduction of the acrylamide for two years. In August, 1997, he had a weakness of the lower extremity with loss of body weight of 15 Kg which progressed to paresthesia of the limbs, the loss of vibration sense, difficulty in walking, speech disturbance, dysuria, constipation, difficulty in erection and swallowing difficulty. Neurologic examination showed a complete loss of vibration and position sense, and loss of deep tendon reflex in all the limbs. There was a contact dermatitis-like skin lesion on the feet. Electrodiagnostic test showed a generalized polyneuropathy of moderate degree involving motor and sensory component of peripheral and cranial nerves. After 8 months' cessation from exposure and supportive care, his symptoms were completely resolved, but abnormality in electrodiagnostic test and vibrotactile perception threshold still persisted, although much improved than before. There were 11 workers in the same workplace of the case, producing acrylamide by microorganism as a catalyst. Environmental monitoring of the workplace revealed air-borne concentration of the acrylamide to be from 0.05 to 0.23 mg/m(3) by personal sampling and from 7.65 to 11.65 mg/m(3) by area sampling during filter exchanging process, which far exceeds TLV-TWA of 0.03 mg/m(3). However acrylamide was not detected from the plasma of the workers. Symptom questionnaire showed a markedly higher complain of neurologic symptoms, compared to the workers of the other acrylamide producing companies. Electrodiagnostic test showed a several workers are in the lower normal range without definite abnormality. Vibrotactile perception threshold by Vibratron II showed a significantly increased threshold in the workers of the same workplace compared to the other acrylamide workers and significantly decreased test score of color vision compared to normal controls. After improvement of the production process and strict wearing of the protective device, most workers reported the improvement of clinical symptoms, but vibrotactile perception threshold was not improved. These result suggests the need for the effective environmental monitoring together with a periodic biological monitoring. Development of effective screening test is urgently needed to control and assess the skin absorption of acrylamide.
