Bladder Cancer with Exposure to Benzidine-based Dyes of Dyer

This is the first report on an occupational bladder cancer in Korea. The ease is 41 years old man who worked as a dyer for 17 years at two dyeing factories, which handled nylon and polyester fabrics in Taegu. He was exposed to many kinds of dyes during weighing, mixing, dissolving and dyeing processes. Among many kinds of acid, disperse and direct dyes that he has been exposed to, several dyes have confirmed to contain benzidine-based dyes, one was o-tolidine-based dye, and one was o-dianisidine-based dye. He visited a hospital due to the gross hematuria and urinary frequency in June, 1998, and he had radical cystectomy with ileal conduit diversion. He had smoked a half-pack of cigarette for 20 years. The main risk factor of bladder cancer is smoking, however, he was relatively a light smoker than usual Korean men. He was exposed to the definite occupational carcinogen even though the level was relatively lower than that of dye manufacturers. His age was younger than the prevalence age of bladder cancer caused by smoking. These evidences support that the dyer' s bladder cancer could be related to the occupational exposure to benzidine-based dyes.

A Case of Heat Stroke in an Aluminium Utensil Plant

An 18-year-old man suffered heat stroke after continuous working for 26 hours on his first day in an aluminium utensil plant in August 1998. His job was to put a pressed aluminium utensil on the conveyer before the process of spray painting. The ranges of temperature and relative humidity measured at the local weather-station during the patient' s working period were 23.6-30.2 degrees C, 49-87 % respectively. On arrival the patient was comatose and suffered generalized seizure three times. His rectal temperature was 41.2 degrees C . blood pressure was 90/60 mmHg, pulse was 148 beats/minute and respiratory rate was 28 times/minute. The serum level of AST was 421 IU/L. ALT was 205 IU/L, LDH was 1,160 IU/L. myoglobin was higher than 500 ng/mL. OK was higher than 2,000 IU/L. He recovered consciousness 7th day of admission and discharged after 2 months but cerebellar dysarthria was remained. The patient felt himself several prodromal symptoms of heat stroke and he showed awkward behavior considered to be drowsiness, but the patient and his co-workers neglected them. This case report shows that heat stroke can be occurred in a condition that ambient temperature, humidity, and working load are not extreme. A thorough health education and management concerned with guidelines on salt and water intake, detection of early symptoms of heat-related illness, prompt body cooling and rapid transportation to a hospital is necessary.

Aplastic anemia is characterized by pancytopenia with hypocellular bone marrow. Fifty percent of the cases are idiopathic and the rest are caused by various agents including drugs, chemicals, radiation and viruses. It is difficult to link specific etiologic agents, especially chemicals to the development of aplastic anemia because multiple or unknown exposures may be involved in. Benzene, a common industrial chemical and a component of gasoline, may lead progressively to pancytopenia, aplastic anemia and leukemia when exposed. A petrochemical worker with aplastic anemia was referred to our hospital to evaluate a relationship between the job history and the disease. He worked in the petrochemical plant for .21 years and was exposed to low-level benzene. There was not any other etiologic agent except benzene and this is the case report of aplastic anemia which possibly due to benzene exposure.

Urinary 2-Ethoxyacetic Acid for Biological Monitoring of Workers Exposed to 2-Ethoxyethyl Acetate

This study was carried out to evaluate the association between urinary 2-ethoxyacetic acid(EAA) as a new indicator of biological monitoring for the workers exposed to 2-etho xyethyl acetate(EEA) and independent variables such as the EEA exposure concentration, exposure dose, total EEA absorption amount, work duration, and alcohol consumption. In this study the subjects were the total of 160 drawn from 75 workers who were occupationally exposed to EEA and 85 workers who were not. The results were as follows: 1. In the electronic painting workplace, EEA exposure concentration in the breathing zone of the spray workers was the highest, as showed 2. 88ppm(0. 01-15.1 ppm) and cases of exceeded threshold limit value(TLV = 5ppm) were 12.0% among the total of 75 exposed workers. 2. Four workers(5. 4%) of exposed workers indicated more than 50.0 mg/g creatinine of urinary EAA, the geometric mean urinary EAA in exposed workers was 2.49 mg/g creatinine, and was higher than that of workers who were not exposed. 3. In the spray workers of auto products, the geometric mean of urinary BAA was the highest showing 5.76 mg/g creatinine. 4. The difference of urinary BAA by EEA exposure level was significant and the correlation coefficient between urinary BAA and EEA in air was the highest showing 0. 53O(p < 0.01). In conclusion, the amount of urianry EAA proved to be a good parameter for biological monitoring of workers exposed to EEA.

A Study on the Risk Factors of Low Back Pain in Computer Terminal Operators

Low back pain in computer terminal operators is a very common but important symptom. While the occupational risk factors of low back pain were approved obscurely, there have been few studies on analysis or understanding of occupational risk factors. Therefore, we studied occupational risk factors of low back pain, especially for ergonomic factors as well as general characteristics of workers. We selected 98 subjects for this study, computer terminal operators ordinary using computers in Pusan. We calculated the subjective severity of low back pain of each operator by the Low Back Pain Scoring System. The relation of low back pain score to general characteristics of worker was also analyzed. We analyzed the relationship between low back pain score and individual efforts to prevent low back pain. We measured the ergonomic factors of each computer terminal operator - Trunk Jnclination(TI), Arm Flexion(AF), Elbow Angle(EA), Head Tilting(HT), Knee Angle(KA), using of foot plate and gap between the popliteum and chair. The correlations between these ergonomic factors and low back pain score were then analyzed. And the results are as follows: 1. The prevalence rate of low back pain in computer terminal operators was 39.8% in this study, when low back pain score was converted by the low back pain scoring system. 2. According to general characteristics of workers related to low back pain, the scores were higher in the older age group, the married and the longer work carriers. (p < 0.05) 3. In the analysis of the relationship between low back pain score and individual effort to prevent low back pain, regular exercise, history of schooling for prevention of low back pain, and declaration of intention to participate in low back pain education or exercise were stastistically significant(p < 0.05). And the more severe the low back pain, the higher the intention to participate in low back pain education they had in this study. 4. In the analyses of relationships or correlations between low back pain score and each of the 7 categories of ergonomic factors, only head tilting had a weak reverse correlation with low back pain(r=-0.2999, p < 0.01). From now on, cohort studies for the risk factors of low back pain of workers, who have been forced to work in non-ergonomical positioning at the work site, will be necessary. On the background of these ergonomic studies, the occupational health profession should be required to manage ergonomic working conditions.

Apoptosis of Neuronal Cells Induced by Lead

Lead is a major environmental and occupational neurotoxicant. It has been shown that long-term exposure to a low level of lead impairs the development of brain. For example, it was reported that lead exposure during the childhood causes a learning difficulty and a memory deficit of children. Neurotoxic agents including the lead are believed to cause neuronal death in developing brain by two mechanisms: apoptosis and necrosis. However, the exact mechanism of neuronal death caused by lead exposure is still not known explicitly. In this study, we conducted a study to clarify a mechanism of hippocampal neuronal cell death caused by lead acetate. Hippocampal neurons were cultured for 14-16 days and treated with lead acetate of 1. 10, 100 1 microM concentrations for 12 hours. With the MTT(methyl tetrazolium test) kit, the viability of neuronal cells was measured. Next, in order to examine apoptosis caused by lead acetate, TUNEL (TdT-mediated d-UTP Nick End Labelling) assay was performed. It has been shown that lead acetate reduced the viability of neuronal cells in a dose dependent manner, especially at the concentration of 100 ~M lead acetate. TUNEL immunostain showed brownish signals in the nucleus of apoptotic cells. The proportions of apoptotic cells in the lead?acetate treated group were more higher than those in the controls and increased as lead acetate concentration increased. From above results, it may be concluded that lead in the hippocampal neuronal cells reduced cell viability and one of mechanisms in neuronal cell death by lead appears to be apoptosis.

Feasibility Study of Constructing an Occupational Disease Surveillance System in Inchon

The occurrence of occupational illnesses and injuries has been seriously underestimated in Korea. This underdiagnosis and underreporting of occupational illnesses and injuries subsequently contribute to difficulties in performing effective control of occupational hazards and implementing intervention programs to protect workers. An Inchon model for an occupational disease surveillance system was constructed using multiple data sources. Available data were obtained from medical providers' reports, hospital medical records, medical examination data of employees, and environmental measurement data. This Jnchon model was constructed with the following procedure: 1. assembled multidisciplinary staff 2. geographic targeting was to Inchon 3. established community liaison 4. collected related data to establish the magnitude of problem 5. selected, recruited, and trained case reporters 6. developed mechanisms and guidelines for reporting 7. received, screened, and prioritized case reports 8. analyzed data from case reports and field investigations 9. made and disseminated recommendations 10. evaluated the feasibility of the system. An information system based on the Internet for the occupational disease surveillance was also constructed to support the case reporting of occupational diseases. 133 cases of occupational diseases were collected in Inchon from 1 January to 31 December 1998. These cases included cumulative traumatic disorder, occupational asthma, occupational contact dermatitis, allergic pneumonitis, occupational cancer, noise-induced hearing loss, and pneumoconiosis. These data were different from the previously reported data The occurrence pattern of occupational diseases obtained from this surveillance system will be useful for preventing occupational hazards and for enforcing occupational disease prevention programs. Our experiences in establishing this surveillance system may be also used in other regional settings.

Comparison of Post-Health Care of Workers with Noise Induced Hearing Loss between Medium Sized Industries and a Large Automobile Industry

In order to compare post-health care between medium sized enterprises(50-300 employees) and a large enterprise(an automobile manufacture plant) and to evaluate factors related to post?health care performance of workers with noise induced hearing loss, this study was carried out. One hundred and eighty nine workers were surveyed to the questionnaire on general and occupational characteristics, receipt of the examination result, perception and attitudes toward special health examination, contentedness with post-health care performance on the examination result and reason not to have post-health care performance. The results were as follows: 1. The employee in large enterprise perform post-health care higher than medium sized enterprises and main control method was taking ear protective device. The 27.5% of workers with NJHL did not perform any post-health care. 2. Comparing with large enterprise, medium-sized enterprises had more interview with workers of NJHL and some enterprises performed more health education and more improvement of working environment. But there were a few medium-sized enterprises which did not provide ear protective device and had indifference manager. 3. The variables that significantly related to the post-health care of manager side was receipt of health examination result(p < 0.05), the post-health care of worker side were size of enterprise, age(p <0.05) and receipt of health examination result(p <0. 1).

Factors Associated with Psychoneurobehavjral Outcomes in Workers Exposed to Manganese

The risk posed to human health by environmental manganese (Mn) exposure is unknown. The purpose of this study is to establish if subclinical effects related to Mn exposure and examine the factors influencing psychoneurobehaviral outcomes of Mn exposed workers. This study involved 121 male workers of welding, alloy furnace, and manufacturing of welding stick. Study investigations include: a questionnaire covering demographic characteristics, job and exposure history and medical symptoms and conditions, monitoring workplace air for personal exposure to respirable and inhalable manganese, analysis of blood and urine samples. psychoneurobehaviral test(neurobehaviral core test battery (NCTB), signal change on magnetic resonance imaging (MRJ) scans, minimental state and neurological examination). The main results of this study were as follows: 1. Preliminary results showed that, with increasing manganese exposure, neurobehaviral performance was poorer and signal change on MRI scans increased. 2. The results of examination revealed significant relations on psychoneurobehaviral out-comes (neurobehaviral performance, signal change on MRI scans, and neurological features). 3. In multiple regression, age and educational status as demographic variables and exposure level were significantly related to 4neurobehaviral test. Also exposure level and serum Mn concentrations were positively relatel to signal change on MRI scans and neurological features affected by Mn on the brain (signal change). 4. Psychoneurobehaviral outcomes from Mn exposure were related to neurobehaviral performance, signal change on MRJ scans, neurological features and profile of mood states (POMS), and influnced positively with age, alcohol and smoking history, and duration of Mn exposure, negatively with educational status. Serum Mn concentrations in combination with brain MRI scans, and perhaps a battery of neurobehaviral tests, appear to be the best way to monitor excessive exposure to Mn. These results are consistent with our knowledge on Mn action on the brain and are similar to the type of neurobehaviral dysfunction. They suggest that there may be age, educational status, and life style (alcohol and drinking history) differences with chronic environmental exposure. These findings suggest further evaluation, particularly on relationships between Mn exposure, aging, and susceptibility factors.

Silica exposure results in acute inflammatory response followed by chronic fibrotic change. The mechanism for the maintenance of silica-induced inflammation has not been understood yet. Apoptosis is a morphologically distinct form of programed cell death that plays major role during homeostasis and in many diseases including cancer, acquired immune deficiency syndrome and neurodegenerative disorders. Apoptosis is characterized by cell shrinkage, membrane blebbing and nuclear condensation. To demonstrate the involvement of apoptosis in underlying mechanism for the development of silica-induced pathological changes, this study was designed in vitro and in vivo models. In in vitro study, alveolar epithelial cell line (A549) was stimulated with silica and performed flow cytometry and DNA electrophoresis. In in vivo study, bronchoalveolar lavage (BAL) was done to count the total and apoptotic cells from silica-instilled rats. The results were as follows: 1. Apoptotic cell fraction of silica-treated groups (10 and 50 microgram/cm2) was significantly higher than that of control group. 2. Genomic DNA from silica-treated groups (10 and 50 microgram/cm2 ) showed DNA ladder in agarose gel electrophoresis, while group of 1 microgram/cm2 didn't. 3. Total cell number and apoptotic cell number of BAL fluid from silica-instilled rats (10, 20 and 40 mg/kg) were significantly higher than those of control. 4. Silica induced apoptosis of cells in BAL fluid was confirmed by microscopic observation with nuclear fragmentation. These results suggest that apoptosis may contribute to development of silica-induced pathological changes.

The purpose of this study is to examine the pesticide use and the relationship between keeping the safety rules or use of protective equipments, and poisoning experience. For Puksan province, Choonchun county, we obtained questionnaire data concerning pesticide poisoning and relating factors from 143 people who have farmed and have sprayed pesticides. Important results are follows: 1. The rate of experiences of pesticide poisoning was 33%, and the major symptoms were dizziness(39. 13%), vomiting(20.87%), and visual opacity(11.30%). The most frequent use of pesticide was paraquat(23. 76%), and the next was organophosphates(18.35%). 2. Logistic regression and chi-square analyses showed that duration of spraying per day, following dose limits, and spraying back against wind are significantly associated with poisoning experiences(x2=5.620, p0.018, OR=0.326, CJ=0.109-0.968, OR=0.302, OR=0.097-0.939 respectively). But no associations between poisoning experiences and age, sex, use of protective equipments, and attendance of safety education. Based on these data, despite limitations in exposure assessment and recall biases, it may be necessary to devise a counterplan for development of effective safety education that prevents pesticide poisoning such as popular education in CATA pesticide project, as well as prohibition of use of hazardous pesticides that proved to be fatal.

Health Hazards and Diagnostic Methods of Glass Fiber Workers

This study was conducted to evaluate the health hazards and to develop diagnostic methods of glass fiber workers. We examined 40 male glass fiber workers (exposure group) and 57 male non-glass fiber workers (reference group) with a questionnaire, physical examination, chest x-ray and pathological examination in Mar, 1997. Also we examined 65 male glass fiber workers (exposure group) and 42 male non-glass fiber workers (reference group) with the same methods also we did some energy-dispersive x-ray analyses with a scanning electron microscopic examination in Sep. 1997. Most of the clinical symptoms were significantly more frequent among the exposure group than the reference group. Coughing (32.5%), itching of the nose (30.0%), irritation of eyes (27.5%), irritation of the nose (25.0%) and sputum (22.5%) were the major symptoms among the exposure group in Mar. 1997. Only itching of the nose was significantly more frequent in Sep. 1997. No cases of pneumoconiosis were observed among the groups and there were no differences in chest abnormalities between the exposure group with the reference group on both examinations. The prevalence of dermatosis among the exposure group was 20.0% (8 cases) and the cumulative prevalence was 72.5% (29 cases) in Mar. 1997. The prevalence of dermatosis among the exposure group was 23.1% (15 cases) and the cumulative prevalence was 58.5% (38 cases) in Sep. 1997. Onset of dermatosis among the exposure group was most frequent within one month after handling. The frequent sites of skin lesions were the hands and arms on both examinations. Glass fiber induced skin lesions can be diagnosed by the scotch tape method or KOH mount and then can be examined under the light, polarizing and phase-contrast microcopies. Glass fibers can be identified by some energy-dispersive x-ray analyses with a scanning electron microscopic examination.

The Work-Related Cases among Bronchial Asthma Diagnosed at a University Hospital in Incheon

Bronchial Asthma could be developed by occupational origin. 4-16% of bronchial asthmas are known to be related to occupational exposure. However, occupational asthma has not been reported from the periodic occupational health examination, but reported through case report from clinicians. The authors analysed hospital records to find out work-related bronchial asthma and their occupational characteristics at a university hospital in Jnchon. All records which were confirmed as bronchial asthma in adult by an allergist from June, 1996 to May, 1998 were selected. Through record reviewing, any suspected work related asthma was selected and the interview was done by phone or at the out-patient clinic. The possible cases were confirmed by additional tests including specific bronchial provocation test. Sixteen(3. 9%) out of 411 bronchial asthma were confirmed as work-related asthma. Eight cases out of 16 worked at furniture industry, three worked at dye manufacturing and two worked at musical instrument manufacturing industry. Others worked at farm and machinary industry. The confirmed asthmogen were toluene diisocyante(8), reactive dye(3), and grain dust(1). None of the cases was detected as work-related asthma in the periodic health examination. Twelve out of sixteen occupational asthma patients discontinued their work. The results suggested that many patients with occupational asthma were not reported and the periodic health examination was not an effective system for detecting the workrelated asthma. Thus, setting up the surveillance system involving allergist or pulmonologist would be important to detect and prevent occupational asthma.

CYP2E1 Genetic Polymorphism relating to Styrene Metabolism of Korean Workers

The goal of this study is to observe the associations between the metabolic phenotype by personal exposure and urinary metabolites and genetic polymorphism of CYP2E1 which is known to be related with styrene metabolism. To complete this study, the author executed a battery of tests on 46 workers who were working at laminating department of fiberglass- reinforced plastics (FRP) industry located in Pusan and Kyungnam area during April to June 1998. Those were - (1) personal exposure assessment with organic vapour monitor and gas chromatography. (2) measurement of urinary metabolites - mandelic acid (MA) and phenylglyoxylic acid (PGA) - with high performance liquid chromatography (HIPLC), (3) CYP2E1 genotying with PCR and restriction fragment length polymorphism (RFLP) using Dra I and Rsa I, and (4) questionnaire survey for some individual characteristics. Study subjects were composed of 32 men and 14 women, and whose average age was 39.4 years, average tenure was 7.7 years. Each concentration expressed by geometric mean(range) was as follows; air styrene 15.6(3.1-81.0) ppm, urinary MA 187.8(36.8-1007.2) mg/g creatinine, PGA 232.8(46.8-1075.7) mg/g creatinine. Correlation coefficients between air styrene were MA 0.54, PGA 0.37, MA+PGA 0.54 (p < 0.05). The relative frequency of CYP2E1 mutant allele was 45.7%(Dra I 43.5%, lIsa 1 37.0%), and homozygous mutant type (M/M) was not observed. The value of (geometric mean of (air styrene/urinary metabolites)) x 1000 according to genotype was significantly higher in mutant type than wild type (p<0.05), as in case of MA, mutant type 106.4 and wild type 84.4, and in case of MA+PGA, mutant type 84.4 and wild type 55.6. The value of air styeneTLV-TWA/urinary metabolitesBEI was used as a cut-off value of classifying phenotype. That is, the value of air styeneTLV-TWA/urinary MABEI >or= 0.063 and air styreneTLV-TWA/urinary MA+PGABEI >or= 0.048 was classified as poor metabolizer, and, the value of air styreneTLV-TWA/urinary MABEI~ < 0.063 and air styreneThV~A/urinary MA+PGABEI < 0.048 was classified as extensive metabolizer. As the result, the frequency of poor metabolizer was higher in mutant type than wild type with no statistical significance (p > 0.05), as in case of MA, mutant type 66.7% and wild type 48.0%, and in case of MA+PGA, mutant type 81.0% and wild type 56.0%. These results suggests that CYP2E1 mutant allele has a tendency toward the poor metabolizer. This study has several limitations as small sample size, and no considerations on work intensity, alcohol habit, obesity, etc which can affect styrene metabolism. However, this study is of value because this is first study to propose the fundamental data about associations between exposure level, biological monitoring, and CYP2E1 genetic polymorphism in Korean workers dealing with pure styrene. To improve accuracy of the study, that means, to applicate the result of this study on the personal risk assessment of styrene workers, larger sample size and consideration for confounders are needed.

A Study on Thyroid Function among Male Workers Occupationally Exposed to Inorganic Lead

This study was performed to evaluate the relationship between biological markers of lead exposure and thyroid functions among male workers occupationally exposed to inorganic lead. 30 male workers exposed to inorganic lead at smelting and battery factories were investigated, and 30 male control workers who were not exposed to inorganic lead occupationally were chosen for this study. The data were obtained using direct interview and assessment of biological markers of lead exposure and thyroid indices. As biological markers for lead exposure, blood ZPP. blood lead, urine lead and hemoglobin were measured. As thyroid indices. TSH and FT4were determined. The results of the study were summarized as follows 1. Levels of ZPP, blood lead, and urine lead were significantly higher in lead-exposed group than in nonexposed control group(p<0.01 all). Hemoglobin level was significantly lower in lead-exposed group than in nonexposed group(p<0.01). 2. Study subjects had normal values of TSH and FT4 But TSH level was insignificantly lower in lead-exposed group than in nonexposed control group(p=0.11) and FT4 level was significantly lower in lead-exposed group than in nonexposed group(p<0.01). 3. As the result of simple regression analysis in lead-exposed group. FT4 level was negatively correlated with the amount of smoke, blood ZPP, and blood lead(p < 0.05 all). 4. In lead-exposed group, FT4 was used as a independent variable and age, the amount of smoke, blood ZPP, and blood lead were entered as a dependent variables in the model. As the result of multiple regression analysis with stepwise selection method, the amount of smoke and blood ZPP were negative predictors of FT4(p=0. 04, p=0.09, respectively). In conclusion, levels of TSH and FT4were lower in lead-exposed group than in nonexposed control group. In lead-exposed group, FT4 level was negatively correlated with amount of smoke, blood ZPP, and blood lead in simple regression analysis. To clarify this relation, further research between lead exposure and thyroid function may be required for worker with long-term lead exposure or high lead exposure.

The Effect of Noise Exposure and Age on the Changes of Group Mean Hearing Threshold Level: Annual Follow-up Studies for Seven Years

This study was carried out to evaluate the effect of noise exposure and age on the changes of group mean hearing threshold level(HTL) over 7-year period. For this purpose, consecutive annual audiometric tests were performed from 1991 to 1997. among 718 male employees of a metal product manufacturing factory. The subjects were divided four groups as follows according to noise level category(NLC). NLC-I officer, exposed noise level was under 60 dB(A) of time weighted average(TWA) NLC-JI technical assistant or engineer, they exposed to workplace noise occasionally NLC-IJJ worker, exposed noise level was below 85 dB(A) of TWA. wore hearing protection device(earmuff or earplug) NLC-IV worker, exposed noise level was over 85 dB(A) of TWA, wore hearing protection device(earmuff and earplug). The results were as follows 1. The improvement of group mean HTL was continued until the fifth year, showing the peak at the third year. The magnitudes of this learning effect were 1.5 - 4.6 dB. 2. The mean HTL of each age group tended to increase after the third year and the tendency was more prominent at 4000 Hz. 3. In noise exposed group(NLC-II, III, and IV), mean HTL increased from the third year after decreasing, meanwhile, in noise free group(NLC-I). it was few changed for the entire period. Among the noise exposure group, the mean HTL of NLC-IV was lower than that of NLC-III and NLC-IV. 4. After learning effect, the mean increase of HTL in noise free group(NLC-I) was 0.4-1.7 dB that suggests aging effect, and that in noise exposure group(NLC-ll, III, and IV) was 0.9 -4.1 dB that suggests noise effect. 5. Statistical analysis of the general linear model implicated that the effect of age was statistically significant at 500, 3000, 4000 and 6000Hz. and the effect of noise exposure was statistically significant at all frequencies except 6000Hz. However, the age * noise interaction was not significant at all frequencies. From these results, it was concluded that the effect of age and noise exposure seems to affect the mean HTL independently and these two factors contribute to an additive effect for the mean HTL change. Furthermore, more concerns should be needed for hearing conservation of low level exposures without any specific protection.

Effect of Desferrioxamine on Silica-Induced Cytotoxicity of A549 and Fibroblast Proliferation

The inhalation of crystalline silica results in the production of reactive oxygen species(ROS). Among these ROS, hydroxyl radical( OH) is believed to be the most reactive one. OH is generated in reaction between superoxide and hydrogen peroxide catalyzed by transition metal, especially iron. Therefore iron should be important in the bioactivity of crystalline silica. Desferrioxamine, a iron chelator, may be protective in silica-induced pulmonary reaction. To test this assumption we investigated the protective effect of desferrioxamine on lipid peroxidation of cell membrane, cytotoxicity, production of proinflammatory and chemotactic cytokine and fibroblast proliferation by crystalline silica in vitro model. The results were as follows: 1. Fenton activity of silica and asbestos was significantly higher than that of control. Fenton activity in crocidolite was higher than silica at the same dose. This result correlated with iron content of dust. Fenton activity of silica and crocidolite was decreased by preincubation of silica with desferrioxamine. 2. Silica induced a dose-dependent increase of MIDA concentration in lung epithelial cell lysate dose dependently. Marked decrease of MDA was observed in desferrioxaminetreated silica group compared with untreated group. 3. As concentration of stimulated silica, silica?induced cytotoxicity was increased. There was significant decrease of cytotoxicity in desferrioxamine?treated silica group compared with untreated group. 4. a-quartz augmented the production of TNF-a and IL-8 from A549 cell. While desferrioxa-mine suppressed the release of cytokines. 5. Supernatant of silica-cocultured A549 cell induced a significant proliferation of fibroblast, which desferrioxaime blocked this proliferation. From these result, we concluded that desferrioxamine has a protective effect on silica induced pulmonary reaction.
