Discriminating Function of Preemployment Medical Examination and Necessity of Changing over to Preplacement Medical Examination in Korea

In Korea, every worker should undergo a preemployment medical examination according to Law for Occupational Safety and Health. But evaluation of it has not been previously carried out. Thus a telephone survey of 213 establishments of Inchon area for evaluation of preemployment medical examination in Korea was conducted. One hundred and fourteen companies(46.5% of all the subjects) were examined medically on preemployment, 47 establishments(41.2% of this study subjects) reevaluated workers by each establishment's medical criteria, and only less than half of diseased workers(DM 38.6% ; hypertension 42.1%, HBV carrier 28.0 % ; active pulmonary tuberculosis 7.9 %; back pain 12.3 %, and noise induced hearing loss 18.4 %) were employed. Employee's fees for preemployment medical examination were covered by 53.5 % of all the subjects. From our survey, it is clear that preemployment medical examination plays discriminating role on the employment of workers based on health status. We thus proposed to change over from preemployment medical examination to preplacement examination based on the evaluation of essential job functions and reasonable accommodation.

Factors Affecting the Complaints of Subjective Symptoms in VDT Operators

In order to investigate symptoms prevalence related to visual display terminal (VDT) syndrome, and to evaluate the variables affecting the development of the VDT syndrome, a questionnaire survey was conducted on 119 (92 men and 27 women) VDT operators who had been engaged in one plant design company in Seoul. The results were as follows : The average age of questionnaire respondents was 30. The average years of service were 4.3 years, and the working hours per day with VDT for half of them were more than 4 hours with the average being about 4.7 hours. The majority of subjects (71 persons) were working at computer aided design (CAD) or computer programming tasks. Subjective symptoms of VDT syndrome were divided into 4 groups with musculoskeletal, eye, skin, and industrial fatigue according to their major affected organ. 4 high incidence of complaints of eye discomfort was observed. In the 3 subgroups of industrial fatigue symptom, the highest average group score was accounted for by 'dullness and sleepiness', followed by 'difficulty in concentration', 'bodily projection of fatigue' in the order of sequence, suggesting the heavier mental stress of VDT work rather than physical burden. Relationship among these 4 groups of symptoms showed statistically significant correlations one another. This result suggests that VDT syndrome is a multi-organ disease composed of these 4 symptom groups which were commonly caused by the use of VDT. Multiple regression analysis were used to examine the relationship between risk factors and these symptom groups of VDT syndrome. As the result of analysis, the most important risk factor for VDT syndrome was the average daily VDT working hours. The longer was VDT working hours, the higher was symptom prevalence of VDT syndrome. In addition, daily total working hours including VDT working hours and shift of work also affected the health of VDT users. In order to protect workers from VDT syndrome, it is urgently required to implement standard management recommendations including restriction of VDT working hours and allowance of more sufficient resting time for VDT workers.

Cumulative Trauma Disorders among Telephone Directory Assistance Operators

To evaluate the prevalence of cumulative trauma disorders in relation to ergonomic evaluation of their work, 260 female directory assistance operators employed in three branch facilities of the nation-wide telecommunication company. Ergonomic evaluation of the work status was done by two industrial hygienists through inspection. Workers were surveyed with standardized self-administered questionnaire and examined by family physicians and an orthopaedician for cumulative trauma disorders. Laboratory examination of serum aspartate aminotransferase(AST), creatinine phosphokinase(CPK), aldolase and rheumatoid factor was done and X-ray of cervical spine and both shoulders was taken from all subjects and nerve conduction velocity test was done on 57 subjects with suspected nerve entrapment cases. Cumulative trauma disorders were categorized into 3 category according to work-relatedness of the disorders. On ergonomic evaluation of the working posture and work environment, improvement of work posture revealed inappropriate postures and devices. Mean age of the subjects were 39 years old and more than 80% graduated high school or above. Ninety-eight percent of the workers employed in the company for more than 15 years and 74.6% of the workers worked at the VDT job for more than 4 years. Mean daily work hours was 8.0 hours per day and mean break hours was 90.5 minutes per day with mean extra work hours of 10.7 hours per month. Mean daily calls were 1128.6 calls in maximum and 864.8 calls in minimum. On laboratory examination, 13 (5.0%) workers had abnormal AST, 53(20.4%) had abnormal CPK, and 1 (0.4%) had abnormal aldolase level. On x-ray examination of cervical spine, 111 (42.7%) had loss of or reversed normal cervical lordosis, 10(3.8%) had narrowing of intervertebral space. Nerve conduction velocity test was abnormal in 11 (19.2%) among 57 workers. Among disorders categorized as work-related, 131 (50.4%) were diagnosed to have fatigue myalgia of levator scapulae, 108 (41.5%) had cervical strain or sprain, 47 (18.1%) had fatigue arthralgia of phalangeal joints, and 47 (18.1%) had probable carpal tunnel syndrome, in 10 (3.8%) of those diagnosis was confirmed electrophysiologically. Age, seniority, work duration at present job, workload nor location of facilities were associated with the prevalence of symptoms nor specific diagnoses on simple statistical analysis and multiple logistic regression, which deems to be due to highly selected and homogeneous characteristics of the study subjects. Above results shows that cumulative trauma disorders are very prevalent among directory assistance operators and elaborate effort is needed to reduce the cumulative trauma disorders among the operators.

A Study on the Lead Exposure of Workers in Secondary Smelting and Litharge Making Industries

Two hundred and six male lead workers (86 lead workers from secondary smelting industries and 120 lead workers from litharge making industries) and 212 office workers who were not exposed to lead occupationally were studied to investigate the effect of lead on the health of lead workers in high risk operation to lead. Blood lead (PbB), zinc protoporphyrin in whole blood (ZPP), delta-aminolevulinic acid in urine (ALAU), Hematocrit (Hct) and hemoglobin (Hb) were selected as an index of lead exposure. Questionnaires of 15 lead related symptoms were provided to all study subject and filled up by themselves and reconfirmed the symptom answers by interviewing medical doctor. While the mean value of PbB, ZPP and ALAU of lead workers in secondary smelting industries were 62.7+/-14.6 microgram/dl, 103.1+/-60.5 microgram/dl and 7.4+/-10.3 mg/liter, those of lead workers in litharge making industries were 58.8+/-14.1 microgram/dl, 74.2+/-45.2 microgram/dl and 5.3+/- 7.9 mg/liter respectively, and there were statistically significant difference of mean values between two groups. Simple linear regression analysis of PBB on ZPP and ALAU shoved the steeper slope in secondary smelting industries than litharge making industries. Higher symptom prevalence rates of lead exposed group were observed in lead colic related symptom (S4) and neuromuscular symptoms compared with non-exposed workers. The prevalence of neuromuscular symptoms only showed dose-response relationship by the increase of PBB and ZPP. But more statistical significances were observed in neuromuscular symptoms by the increase of PBB than ZPP. On stepwise multiple regression using total symptom score as a dependent variable and PBB, ZPP, ALAU, Hct, Hb, age and work duration as a independent variables only ZPP and age contributed to the lead related symptoms.

A Study on Factors Related to NO Synthesis by Mercurial Compounds in the EMT-6 cell

The effects of several factors on the nitrite synthesis were observed in the culture of EMT-6 cells which are originated from mammary adenocarcinoma of Balb/c mouse. The cells were cultured in Dulbecco's Modified Eagle's Medium containing 10% fetal calf serum. Amounts of nitrite in the culture media after 24 and 36 hours of culture were about 2 fold, and 3-fold of those measured after 12 hours respectively. There were very close associations between the amounts of nitrite measured in the culture media. A significant nitrite synthesis by EMT-6 cells occurred when IL-1 was added to the culture medium with other cytokines as IFN gamma or TNF alpha . One of each cytokines were less effective as an inducer of nitrite than the combinations of cytokines. When mercury chloride or cinnamate was added in the culture medium, the nitrite synthesis was dose-dependently decreased by the concentration of these materials. The viability of EMT-6 cells were kept on 95% or above in 36 hours after beginning of culture without any specific additives except cytokines. While after 48 hours it went down to 85% or less. These viability were decreased by the prolongation of culture time (48 hours or more), the addition of TNF alpha to cytokine mixture, and the higher concentrations of mercury chloride or cinnamate to culture medium. Simultaneous addition of the equimolar dose of selenium completely prevented mercurial compounds-induced inhibitions of nitrite syntheses. But the single addition of selenium neither influenced the viability of cells nor the productions of nitrite. These results suggests that the disorder of cell mediated immunity by mercurial compounds could be related to the inhibition of nitric oxide synthesis and selenium decreased the cytotoxicity of mercurial compounds.

Changes of Renal Cadmium Accumulation Levels and Urinary Cadmium Excretion Levels Induced by Long-term Cadmium Exposure in Rats

This study was performed to compare the cadmium accumulation levels in kidney with urinary cadmium excretion levels according to the duration of cadmium exposure in rats. Total 120 male rats, 6 weeks of age, were administered 0.5 mg/day of Cd to subcutaneously, intraperitoneally and orally 6 times per week for 12 weeks. The animals were sacrificed in the 1st day of 2nd, 4th, 6th, 8th, 10th and 12th week after beginning of cadmium administrations. Body weight, hematologic values, enzyme activities, renal Cd levels, urinary Cd and proteins were calculated and each value was compared according to the Cd administration routers. The Cd accumulation levels in the kidney of rats increased remarkably for 6-8th weeks which were experimented and urinary Cd excretion levels and urinary protein levels showed the tendency of increasing with the Cd accumulations in kidneys, especially in the subcutaneous injected rats. On the other hand, there was a significant positive correlation among the total Cd administration, the Cd accumulation in the kidneys, the urinary Cd excretion and protein.

Relating Factors of Industrial Accident

This study was conducted to estimate the causing factors of industrial accident, and to prepare the fittable control program for accident through the evaluation of views of the patients who suffered from accidents. From January 5 to February 6 1993, questionnaire survey was executed on 166 persons who were injured and covered by the insurance under industrial accident compensation law at four university hospitals in Pusan. The results were as follows : 1. Most of subjects worked in the manufacturing (56.6%) and construction(29.5%), and in the small factory less than 49 workers(56.0%). They were thirties and forties(63.8%), less than 3 years tenure(53.6%), habitual drinker(69.2%). And, 72.3% worked for above 9 hours per day, 53.6% worked as near clerks, 89.2% thought that work is rather suitable to his own personality. 2. Self-recognition for the causing factor of industrial accident was the other's malfunction(19.3%), carelessness(15.1%), lack of education for safety(15.1%), and they thought the accident occurred by mainly some defect of establishment and working environment(34.9%), unskillfulness(22.9%), self-carelessness(22.3%). 3. The interests and recognition on safety was insufficient, 88.6% require job education, but 53% pointed out the necessity for strengthening of present status-rather incomplete with relying on self-education (40.9%), no previous participation in safety education (29.5%), etc. 4. For the kind of accident, fall-down(16.3%), crash(13.3%), falling(12.7%) was frequent, head(24.7%) and fracture(41.0%) was frequent for the part and type of injury, respectively. And, they had more injured by cooperative work (82.5%) than single work. As results, improvement of working environment and strengthening education program would be emphasized for the effective control of industrial accident. And, there should be prepared the specific prevention program for the small scale industries.

Symptom Prevalence of Work-Related Musculoskeletal Disorders and Related Factors among Bank Workers by Visual Dispaly Terminal Use

The purpose of this study is to investigate the relationship between the self-reported musculoskeletal symptoms and related factors among VDT operators in bank. The subjects were 950 women workers. The characteristics of subjects were 26.6 years of average age, 5.7 years of average VDT use duration and 6.5 hours of daily VDT use time. The results were as follows. The prevalence of self-reported musculoskeletal symptoms more than severe in Likert scale were 51.4%, 43.9% at the shoulder, 38.3%, 31.8% at the lower back, 38.0%, 31.3% at the neck, 31.2%, 25.2% at the upper back, 21.7%, 16.5% at the wrist and 13.6%, 8.8% at the fingers (while on duty, after home-coming). General characteristic variables affecting the musculoskeletal symptoms were married, number of child and daily house-working time. Work-related factors affecting the musculoskeletal symptoms were VDT working career, daily working time, daily VDF use time, VDF use time without rest, daily work-load and regular rest. As the result of the multiple logistic regression analysis, daily working time, daily VDT use time and VDT working career were significant variables accounting for the musculoskeletal symptoms.

A Study on Immunity of Welders

The authors examined human immunity indicators such as CD4, CD8, Natural Killer (NK) cell, IgG, IgA, and IgM, and heavy metals such as Pb, Cr, Cd to evaluate the health hazard of welders. We selected total 126 male subjects, 60 welders who had more than 5 year working history as an exposure group, and 66 office workers and medical college students as a control group. This immunity evaluation approach is the first newly designed study that has never been tried in past studios on welder's heath evaluation. And the results were as follows. 1. Total lymphocyte count of peripheral blood was significantly higher in welder group, 2,615+/-650 ea/microliter compared with 2,368+/-681 ea/microliter in control group (p<0.05). 2. The proportion of CD4, CD8, NK cell in welder group was 37 5+/-9.4%, 24.0+/-8.8%, and 21.0+/-9.5%, respectively, and 35.5+/-6.5%, 25.0+/-6.9%, and 19.6+/-8.8% in control group. CD4 and NK cell were slightly higher, and CD8 was slightly lower in welder group. But there was no statistical significance between two groups (p> or =0.05). 3. IgG, IgA, and IgM in welder group were 15.9 mg/dl, 2.7 mg/dl, and 1.5 mg/dl, respectively, and 15.6 mg/dl, 2.9 mg/dl and 1.7 mg/dl in control group. There was no statistical significance between two groups(p> or =0.05). 4. Blood lead, cadmium and chromium concentrations were 4.2+/-1.8 microgram/dl, 0.4+/-0.38 microgram/dl, 1.2+/-0.6 microgram/dl in welder group and 3.2+/-1.7 microgram/dl, 0.44+/-0.29 microgram/dl, 1.1+/-0.6 microgram/dl in control group, respectively, and these value were within normal ranges.

Factors Affecting on Hospital Workers' Stress Symptom

Nowadays, stress is one of the most important factors that affect workers' chronic diseases. In the case of China, medical demands increased under the national health service system after the market economy was introduced. These socioeconomic changes have affected the hospital workers' stress in China. We surveyed 428 hospital workers in Jilin Province in China from March 18 to March 29, 1996. The results were as follows ; 1. There were more women(67.8%) than men(31.1%) and those who graduated from technical school were 54.2%. The workers whose monthly income ranged from 400 to 499 Yuen were 37.9% by Chinese currency. For occupational characteristics, there were 36.4% doctors and 30.1% were nurses among the hospital workers. 29.9% had worked for 10~20 years in the hospital and 80.1% said their workload was moderate. 25.9% were satisfied with their jobs and 33.2% had good relationships with superiors and 37.4% had good relationships with co-workers. 2. For health habits, 22.7% were current smokers and 51.2% were scarcely drank. among this subjects. Those who ate breakfast almost everyday were 87.1% and the workers who ate between meals almost everyday were 50.2%. 83.8% slept for 7~8 hours at night. Only 14.0% of the workers exercised regularly were only 14.0% and 18.5% were obese. 3. Hospital workers' stress symptom was significantly related to age and monthly income. The workers who were 19 years old or less or 40 years old or more and had higher monthly income showed higher stress symptoms than those who were between 20-39 and had lower monthly income. Higher stress symptom was also significantly related to the heavy workload, the dissatisfaction with their job an4 the bad relationship with their superiors and co-workers. 4. Higher stress symptoms were significantly related to drinking and hours of sleep among the seven health habits. The more frequently the workers' drank, the higher the stress symptoms were and the workers who slept for 7~8 hours at night had lower stress symptoms than those who slept for 6 hours or less or 9 hours or more. 5. According to the results of stepwise multiple regression analysis, the factors affecting hospital workers' stress symptoms were monthly income in personal characteristics (R2=0.0227), job satisfaction and workload (R2=0.0742) in occupational characteristics, and drinking and hours of sleep (R2=0.0446) among the seven health habits. Consequently, the most significant factors that had affected hospital workers' stress symptom in China, were job satisfaction and drinking according to the premises of the questionnaire.

Interrelation of Neuropsychiatric Symptom and Neurobehavioral Test among Workers Exposed to Organic Solvents

A cross-sectional study of 118 car painters and controls of 113 workers was performed to investigate the interrelation between self-reported neuropsychiatric symptom and neurobehavioral performance using Scandinavian Questionnaire 16, Zung anxiety, depression scale and Swedish performance evaluation system (simple reaction time, symbol digit, digit span, finger tapping speed). The typical symptoms characteristic of painter's syndrome were not found and no significant association was found between solvent exposure and !total score of neuropsychiatric symptom but neurobehavioral performance of simple reaction time, symbol digit and digit span had association with solvent exposure. No significant associations were found between total score of neurotoxic symptom, anxiety scale, depression scale and the neurobehavioral performance. In factor analysis, the factor of questionnaire 16 measuring function of memory demonstrated no significant association with the factor of neurobehavioral test items measuring short term memory.

Protective Effects of the NMDA-receptor Antagonist (MK-801) for the Brain Injury by Oxygen Free Radical: In the Hyperbaric Oxygen Treatment of CO Poisoned Rat

The effects of NMDA-receptor antagonist (MK-801) were assessed for the oxygen free radical mediated brain (hippocampus) injury with eighty rats which were exposed to carbon monoxide (CO) followed by hyperbaric oxygen (HBO) treatment. Superoxide dismutase (SOD) and malondialdehyde (MDA) were used as parameters of the oxygen free radical reaction. Experimental groups consisted of (1) control group (=breathing with air), (2) CO group (=exposed to CO after air breath), (3) CO-air group(exposed to CO after air breath followed by air breath), and (4) CO-HBO group (=exposed to CO after air breath followed by 3 ATA HBO). Each group was divided two subgroup according to the pretreatment (normal saline or MK-801). CO, CO-air and HBO groups increased in SOD activity as compared with control group. And CO-air and HBO groups increased in MDA as compared with control and CO group. Pretreatment of MK-801 decreased SOD activities significantly (p-value<.05) , but MDA amount not significantly (p-value=.107). These results suggest a useful protective effect of NMDA-receptor antagonist (MK-801) in CO induced hippocampal injury mediated by oxygen free radicals.

Subjective Symptoms of Fatigue in Pharmacists

In order to investigate industrial fatigue of pharmacists, the questionnaire survey for subjective symptoms and its related factors was carried out on 435 pharmacists who were managing pharmacies in Seoul. The Questionnaires were composed of three groups of 10 items each, representing dullness and sleepness(I), difficulty in concentrations (II) and bodily projection of fatigue (III). The results were as follows : 1. The complaint rate of fatigue was the highest in 'eye strain' and followed by 'whole body tires', 'apt to forget', 'leg feel heavy', 'feel drowsy' and 'feel like tying' in the descending order. 2. The average weighted score of fatigue complaint was the highest in dullness and sleepness group(I) and followed by difficulty in concentrations group(II) and bodily projection of fatigue group(III) in the descending order, suggesting the heavier mental stress rattier than physical burden of the pharmacists work contents. 3. The average weighted scores of fatigue complaints were significantly higher in female and 0-3 off-duty days per month group than those in male and more than 4 off-duty days per month group. 4. The average weighted scores of fatigue complaints were significantly different among the characteristic groups of counselling postures, noise and using chairs. 5. The groups with the high quality of sleeping, regular physical exercise and diet, or satisfied with their job and income had significantly lower average weighted scores of fatigue complaints. 6. The 42.5% of the subjects complained feeling heavy chronic fatigue. And the significant differences for average weighted scorns were noted among three groups, representing 26.64 in feeling heavy chronic fatigue, 18.23 in feeling light chronic fatigue, and 9.50 in feeling no chronic fatigue.

Many acute and chronic lung diseased including pneumoconiosis are characterized by the presence of increased numbers of activated macrophages. These macrophages generate several inflammatory cell chemoattractants, by which neutrophil migrate from vascular compartment to the alveolar space. Recruited neutrophils secrete toxic oxygen radicals or proteolytic enzymes and induce inflammatory response. Continuing inflammatory response results in alteration of the pulmonary structure and irreversible fibrosis. Recently, a polypeptide with specific neutrophil chemotactic activity, interleukin-8 (IL-8), has been cloned and isolated from a number of cells such as: monocytes, macrophages and fibroblasts. IL-1 and/or TNF-alpha preceded for the synthesis of IL-8, and we already observed high level of IL-1 and TNF-alpha in vivo experiments. So we hypothesized that IL-8 might play a central role in the pathogenesis of pneumoconiosis. In order to evaluate the clinical utility of IL-1, IL-8, TNF as a biomarker in the early diagnosis of pneumoconiosis, we investigated the increase of IL-1, 8, TNF in the pneumoconiotic patient and the significant (p<0.05) correlation between IL-8 level and progression of pneumoconiosis.

Reactive airways dysfunction syndrome (RADS) is a syndrome as a persistent bronchial hyperreactivity with asthmatic dyspnea which occurs after one or more inhalation exposures to a high concentration of irritant gasses, smokes or vapors in subjects who had preciously had no respiratory disease. We report a case of a 34 years old female worker suffering from reactive airways dysfunction syndrome who had been taking charge of guest room cleaning at a condominium. 7 months before, she inhaled high concentration of chlorine gas in confined and poor ventilated working space of bathroom at the condominium. As she was washing a bathroom with a bleaching agent (6 % sodium hypochlorite) without dilution, dyspnea, dizziness, headache, nausea, and coughing developed after work. She still suffer from dyspnea and coughing. All functional tests were normal except for methacholine challenge test which reveals bronchial hyperreactivity. She has some symptoms of dyspnea and coughing when exposed to perfume, cold air, and heavy work.

RELATED FACTORS OF DIAGNOSIS OF CHRONIC CARBON DISULFIDE POISONING

The purpose of this study was to compare the characteristics between chronic CS2 poisoning group and non-poisoning group, to find out major factors for decision of chronic CS2 poisoning, to describe the characteristics of cerebral infarctions, and to find out the related factors of cerebral infarctions. The study subjects were 220 ex-employees from W company and the study period was from August, 1993 to November, 1995. The proportion rate of chronic CS2 poisoning was higher in older age groups and longer work duration groups. The rate differed by cerebral infarction, microaneurysm, other retina lesions, polyneuropathy, psychiatric lesions, and sensorineural hearing loss. The cerebral infarction was found to be mainly multiple and the frequently occurred locations were periventricular white matter, frontal lobe and basal ganglia. The logistic regression for chronic CS2 poisoning showed that the major factors were microaneurysm, cerebral infarction, polyneuropathy, other retina lesions, diastolic blood pressure, and sensorineural hearing loss. Among the approved group 117 patients were followed up continuously at the outpatient clinic, 62.4% of the patients had hypertension, 61.5% had polyneuropathy, 52.1% had cerebral infarction, 35.0% had hyperlipidemia, 29.9% had liver disease, 15.4% had psychiatric diseases, 6.0% had angina, and 4.3% had anemia. For the hypertensive patients, 1.9 kinds of antihypertensive drugs were needed to control the blood pressure, and the BP decreased from 164/101 mmHg to 131/81 mmHg.
