The Relationship between Shift Work and Work-related Injuries among Korean Workers

OBJECTIVES
The aim of this study was to evaluate the relationship between shift work and work-related injuries in a representative sample of Korean workers.
METHODS
We used data from the Korean Working Condition Survey of 2006. The participants were 7,075 Korean workers 15 to 64 years of age. Logistic regression was employed to estimate the relationship between shift work and work-related injury using the SAS version 9.2 program.
RESULTS
Shift work was significantly related to work-related injuries compared to non-shift workers(OR 2.40, 95% CI=1.65~3.48). When personal and occupational risk factors were accounted for, shift work had a statistically meaningful relationship with work-related injuries compared to non-shift work(OR 1.79, 95% CI=1.20~2.66).
CONCLUSIONS
In this study, shift workers appear to have a higher risk of work-related injuries compared to non-shift workers. Work-related injuries in shift workers could be attributed to circadian rhythm disorder and sleep problems. Suitable arrangements that ensure flexible working times are needed for shift workers. Further study will be needed to identify and mitigate work-related injuries among shift workers.

An Assessment of the Needs and the Affecting Factors for Small Scale Enterprise Worker's Health Centers

OBJECTIVES
The aim of this paper is to assess the needs and roles of the Worker's Health Centers employed by small scale enterprises by investigating the requested services made to this type of Worker's Health Center.
METHODS
The population of this research was taken from 1,145 business owners or managers of manufacturing companies employing less than fifty people in six industrial estates. Stratified sampling was used. Individual interviews were performed to assess the conditions and needs of these enterprises using a structured questionnaire and professional interviewers.
RESULTS
According to the questionnaire survey, 941(82.2%) of the enterprises wish to use a Worker's Health Center. 946(83.3%) of the enterprises preferred an 'Expert visit to the enterprise or mixed' for the Delivery type. 486(42.4%) of the enterprises preferred the 'in the center of industrial complex' for the location of the Worker's Health Center. 789(69.1%) of the enterprises preferred 6-10 PM for the available open hours. A multiple regression analysis revealed a significantly positive association regarding the intention of using a Worker's Health Center and the Gyeonggi-Banwol (OR 2.49) or Gwangju-Hanam(OR 4.24) for the region. Compared to enterprises employing 1-4 worker, 30-49 worker enterprises show a 3.33 fold preference (95% CI 1.80~6.12) for the use of a Worker's Health Center.
CONCLUSIONS
Generally, the intention regarding the use of a Worker's Health Center was high, Considering the time and spatial limits inherentin small scale enterprises, the 'expert visit to the enterprise' paradigm is important. It should be noted that when providing the service, a customized approach is necessary, determined by size, region, industry, and scale of the business, through the assessment of the small scale enterprises and the nature of the work environment when designing a Worker's Health Center operation.

Relationship between Workplace Physical and Chemical Hazard Exposures and Mental Health Problems in Korea

OBJECTIVES
Mental health and work efficiency are adversely affected by exposure to physical and chemical hazards in the workplace. This study is the first large scale evaluation of the relationship between physical and chemical hazard exposure in the workplace in relation to the mental health of Korean workers.
METHODS
A national survey of a random sample of workers (n=10,043) was conducted between June and September 2006 in Korea. Physical and chemical hazards in the workplace were considered if exposure occurred over more than a quarter of the time spent at work. Mental health problems were defined as work-related depression, anxiety or insomnia. We analyzed whether physical and chemical hazards were independent risk factors for work-related mental health problems by multiple logistic regression analysis after adjusting for age, gender, employment type, education, occupation, duty schedule, work hours, smoking, and alcohol consumption.
RESULTS
The mean age of surveyed workers was 42 years and 58% were male. A total of 7.8% of workers had work-related mental health problems. Exposure to high temperature, mist, fumes, dust, chemicals, and radiation were associated with an increased risk of work-related mental health problems.
CONCLUSIONS
This study reaffirmed that physical and chemical hazard exposure in the workplace significantly elevates the risk of work-related mental health problems.

Association between Physical Workload and Work-related Back Pain: A Nationwide Study

OBJECTIVES
To date, no population-based epidemiological studies have estimated the association between physical workload (standing or walking, carrying heavy loads, carrying people, repetitive hand or arm movements, wearing protective equipment) and work-related back pain in Korea. The purpose of this study was to evaluate the relationship between physical workload and work-related back pain using a nationwide survey.
METHODS
We analyzed data obtained from working men and women age 15 to 64 (n=10,043) who participated in the 2006 national working condition survey of random sample workers in Korea. Using both simple and multiple logistic regression analysis, the association between physical workload and work-related back pain was evaluated.
RESULTS
Carrying heavy loads, repetitive hand or arm movements, and wearing protective equipment were associated with an increased risk of work-related back pain.
CONCLUSIONS
This study identifies that an increased risk of work-related back pain related to the nature and intensity of the physical workload of the general working population in Korea. We also found a strong association between repetitive hand or arm movements and back pain.

Distribution of Blood pressure and Current State of Life Style Modification on Male Workers in a Large Factory: Analysis According to JNC-6 Guideline

OBJECTIVES
To describe the distributions of blood pressure level and risk group, and the current state of life style modification in the population of a large factory.
METHODS
We surveyed 1,088 workers in a large factory from June 2002 to August 2002. Information related to hypertension was collected through a questionnaire and medical examination. We classified the blood pressure level and risk group using JNC-VI criteria, and the current state of life style modification and medication according to the blood pressure level in the entire study population. REULTS: The prevalence of hypertension was 30.1% in all workers. The distribution of blood pressure level was 656 (60.3%) in normal stage, 160 (14.7%) in high normal stage, 190 (17.5%) in stage 1, 67 (6.1%) in stage 2, and 15 (1.4%) in stage 3. The numbers of workers in risk groups A, B and C were 296 (27.2%), 690 (63.4%) and 102 (9.4%), respectively. Among all workers, 432 (39.7%) needed to modify their lifestyle; 106 of whom (24.5%) needed to go on hypertensive drug medication. Overweight subjects were more common in stage 2 than in other stages (p=0.02). The numbers of smokers and low-exercisers were signigicantly lower in the higher blood pressure group than the lower blood pressure group (p<0.01). Alcoholic drinking was evenly distributed in all blood pressure level.
CONCLUSION
The prevalence of hypertension was 30.1% in all workers. The most common stage of blood pressure level was normal, followed by stage 1, high normal stage, stage 2, stage 3. The number of subjects in risk group B was more than that in the other risk groups. We recommend total worker education and the practice of lifestyle modification.

Shiftwork Duration and Metabolic Risk Factors of Cardiovascular Disease

AIMS: To explore the relationship between shiftwork duration and metabolic risk factors on cardiovascular disease in shiftworkers.
METHODS
The study subjects comprised of 226 nurses, from a hospital, and 130 male workers, from a diaper and feminine hygienic material manufacturing firm. The mean ages of the male workers and nurses were 29 and 28.5 years, respectively. The fasting blood sugar, serum cholesterol, blood pressure, height and weight, waist and hip circumferences (only in nurses), and number of step for a shift as indices of physical activity were measured. Using the Korean version of Karasek's job contents questionnaire, the job stress was assessed. Information about the number of years worked, duration of shiftwork, and past medical and behavioral histories, including smoking, were obtained by self-administrated questionnaires. Linear regression analyses were performed, to show the relationships between shiftwork duration and metabolic risk factors, using simple and multivariate models, adjusted for age, smoking, job strain and physical activity. The following criteria were defined: hypertension as a SBP>or160 or a DBP>or=90 mmHg at least once, hypercholesterolemia, as a serum total cholesterol >or=240 mg/dl, obesity as BMI (Body Mass Index) >or=25kg/m2 and central obesity as a WHR (Waist to Hip Ratio) >or=0.85; and the logistic regression analyses, according to years of shiftwork, were performed using simple and adjusted models.
RESULTS
The cholesterol and fasting blood sugar showed increasing trends, but without statistical significances, according to the increase in shiftwork duration of the male workers, although, the increases in the blood pressure and BMI were statistically significant. In the nurses, only the WHR showed a significant increase in relation to the shiftwork duration. In the logistic regression analyses, hypercholesterolemia and obesity showed significant increasing risks according to the number of years of shiftwork (OR=3.32 95%CI 1.27-8.72 and OR=3.21 95%CI 1.24-8.32 respectively) in the male workers, but hypertension showed no significance. In the nurses, only an increased central obesity was significant as a risk factor (OR=1.30 95%CI 1.05-1.62).
CONCLUSIONS
These results provide evidence of the associations between shiftwork and metabolic risk factors for cardiovascular disease, although healthy shiftworker effects might exist in our cross sectional study design.

Influence of Shiftwork Duration on Blood Pressure and Heart Rate Variability in Short Rotating 8-hour Shiftworkers

OBJECTIVES
This study was performed to investigate the chronic health effect of shiftwork on cardiovascular system.
METHODS
We checked blood pressure(BP) and heart rate variability(HRV) at every shift on 134 male workers whose shift type was 8-hour and short rotating of 3 days shift interval. As confoundings, job stress was measured by Karasek's JCQ 49 items questionnaire and circardian type was assessed by morningness-eveningness questionnaire. Smoking and alcohol drinking habit, marital status and past medical history were also acquired. We used the method of repeated measured data analysis with mixed model to show the association between shiftwork duration and BP or HRV.
RESULTS
Average age of workers was 29 years olds(range 25~44). Among them, 77.9 % were current smokers, 50 % were the passive type of job strain in Karasek's model. Mean shiftwork duration was 5.21 years(range 5.4 months~10 years). In circadian type, no one was definitely morning type or definitely evening type. In multivariate analyses adjusted by age, job strain, shift, circadian rhythm and smoking, blood pressure were showed significantly increasing trends according to shiftwork duration in both systolic and diastolic(p<0.05). Heart rate variability were also showed significantly decreasing trends according to shiftwork duration in both parasympathetic and sympathetic functions(p<0.05).
CONCLUSIONS
The results of this studs suggests the chronic negative health effect of shiftwork on cardiovascular system.

OBJECTIVES
This study is to see the transition and pattern of the industrial iureal worker, and to develop the prediction model.
METHODS
The data of the study are based on the samples from data-warehouse of Occupational Safety & Health Research Institute and are summed monthly from Jan 1986 to Dec 1999. This study data used data mart and Meta data from DW in KOSHA. The prediction model of the injured worker in Industry is designed by using a winters time series method after data preparing (i. e. sample, explore, modify) from DW.
RESULTS
Thls predicted model obtained Winters-method multiplicative in exponential smoothing among applied all models, after the tlme series (total 163 months). It showed that the prediction power was 95.5 %.
CONCLUSIONS
In the process of exploring the data, totally the rate of industrial injureal workers reduced, and in the yearly circulation, in February and September the number is the lowest but in June, July, October and November the higher. The number of monthly average injureal workers is 8709 (95 % confidence interval 8277, 9140). From the developed prediction model, since Aug 1999 the industrial injureal worker reduced rapidly in Dec 1999 and first period of 2000. But In second period of 2000 the number of the injured workers is increasing. To conclude, as the total economic situation is becoming better in 2000 than In 1999, its is supposed that the injured workers will increase more than the predictive injured workers because of the increase of production rate and labor force.

OBJECTIVES
Pneumoconisis and noise-induced hearing loss(NIHL) have been reported as main occupational diseases by the Special Health Examination. The Industrial Accident Compensation Insurance has reported various work-related diseases, however, these two diseases occupied almost a half of compensated cases. Therefore, it was not well known about the status of occupational diseases other than pneumoconiosis, NIHL, and cardiocerebrovascular accident(CVA). This study was conducted to analyze claimed cases as an occupational disease, that was requested to the Korea Industrial Safety and Health Agency (KOSHA).
METHODS
The local office of the Korea Labor Welfare Corporation(KLWC) has asked the KOSHA for confirmation of claimed cases as an occupational disease. We analyzed 379 cases requested from KLWC, the Ministry of Labor, employers, unions and occupational health agencies from 1992 to 1999.
RESULTS
Male was 80. 7 % of the requested cases. Their mean age was 42 years old and 75. 5 % of them were more than 35 years old. The requested cases were increased rapidly from 25 cases in 1992 to 108 cases in 1999 and the accept rate was 50. 7 %. The majority of requested cases were respiratory diseases(22.4 %), cancers(18.5 %), Neuropsychiatric problems (14. 5 %), and musculoskeletal problems (13. 5 %). The accept rate was high in reproductive, respiratory, musculoskeletal and digestive disorders and low in neuropsychiatric, renal and otologic problems and occupational cancers. 73. 6 % of them were caused by chemical agents, especially 28. 5 % were by organic solvents. 67 % of them were clinically confirmed at university hospitals. A half of the cases were from KyongIn area, even the request came from the whole country.
CONCLUSIONS
A claim was common in workers whose age was over 35 years old and exposure history was over 10 years. The respiratory diseases and neuropsychiatric disorders were still main problems in occupational health and occupational cancers was increasing even though its accept rate was not high yet.

The small scale industries in Korea are important in occupational health care management due to high accident rate and poor working condition. The authors surveyed the 4,811 small scale industries participated in the 1997 subsidiary program on health care management. The contents of survey were the general characteristics of the factory, the results of workers' general and occupational health examination, the results of workplace environmental monitoring, and the number of occupational accident and injured person at 1995 and 1996. We classified all industries by 67 groups. The prevalence of the general disease was higher in 'Manufacture of other fabricated metal productsj. rManufacture of textiles', 'Manufacture of basic metalsj. In occupational disease, that was higher in FManufacture of metal products', 'Manufacture of basic metals', so in environmental monitoring in 'Manufacture of textiles', 'Manufacture of other fabricated metal products' 'Printfrig' in occupational injuries in 'Manufacture of chemical products', 'Manufacture of basic metals'. However the factories of 'manufacture of motor vehicle', 'manufacture of machine and equipment', 'manufacture of electric product' in the program are large in magnitude, but less important in occupational health management. We observed that the prevalence of general disease, occupational disease, workplace environmental monitoring, occupational injuries of subsidized industries in 1996 were increased compared with those in 1995. and were larger than those of nationwide statistics. These phenomenon shows that the subsidiary health program for participated industries is important and to be urgent. We observed that the major groups in district were different and so the approaching method of each district must be various and different. And the health program manager should focus on the C grade in health examination and the action level in workplace monitoring more than the D grade and the permissible level.

In order to evaluate the genotoxic hazard among workers potentially exposed to low level petrochemical substances, the analyses of micronuclei (MN) and sister chromatid exchanges (SCEs) in lymphocytes were performed in 46 male workers (as exposed group) and 46 nonexposed subjects (as control group). Mean frequencies of MN and SCEs (respectively, 12.9/1000 cells and 6.5/cell) in exposed group were very significantly higher than those (10.2/1000 cells and 5.4/cell) in control group. And there were also significant differences in mean frequencies of MN and SCEs adjusted for age, employment duration, smoking, and drinking between two groups. Median frequencies of MN and SCEs in exposed group were very significantly higher than those in control group. Frequencies of SCEs were higher in smokers than in non-smoker. Frequencies of MN in smokers, however, were similiar to those of non-smoker. Interaction between exposure and smoking on MN and SCEs induction was not observed. The results suggest that there is genotoxic hazard in high risk group like workers handling carcinogens in petrochemical plants and the analyses of MN and SCEs are useful biomarkers for the exposure to hazard substances even at the level below the exposure limit.

To investigate the impact of 2-bromopropane on neuropsychiatric symptoms, 33 exposed workers and 77 non-exposes workers in electronic company were recruited. Neuropsychiatric symptoms were assessed using self-administered questionnaire. Memory disturbance, concentration disturbance, sleep disturbance, understanding disturbance and acute irritation symptom were more frequent in workers exposed to 2-bromopropane. To investigate the association between 2-bromopropane exposure and neuropsychiatric symptoms, total symptom scores of exposed workers were compared with those of non-exposed workers. Mean value in the exposed workers was significantly higher than that of non-exposed comparisons. The mean value of total symptom score of smokers in exposed workers was significantly higher than that of smokers in non-exposed workers. In multivariate regression analysis, exposure status was significant predictor for total symptom score after controlling the effects of age, sex, education, smoking status and alcohol intake. Above findings suggest that 2-bromopropane was significantly associated with neuropsychiatric symptoms. Therefore, it is recommended that further research with objective assessments and long follow-up should be done to confirm the results of this study.

Prevalence of Asbestosis in Korean Asbestos Industry

Asbestos industry has been in operation over 60 years in Korea. However, the prevalence of asbestosis has not been yet reported. With rapid turn-over of workers, previous cross-sectional studies of current workers on the job could not find cases with exposures long enough for the development of asbestosis. This study was done to evaluate asbestosis prevalence of those worksites with operation history of more than 20 years. In total, 139 workers from 5 worksites were examined. Asbestos industries covered in the study include 2 asbestos textile, 1 brake lining, and 2 ship repairing worksites. Chest x-ray was taken from all workers and read by two exports familiar with pneumoconiosis classification according to 1980 ILO guidelines. Those with findings compatible with asbestosis were further checked with high resolution computerized tomography (HRCT). Pulmonary function tests were done according to ATS guidelines, and occupational and previous medical history was taken through a standardized interview. Air-borne asbestos was measured according to NI0SH method 7400. The air-borno asbestos concentrations ranged from 0.2-1.3 f/cc for asbestos textile, from 0.7~l.0 f/cc for brake lining, and from 6.3-7.8 f/cc during asbestos removal at ship repairing worksite. Of the 139 workers 25 had abnormal chest radiographic findings, and 10 of them had findings compatible with pneumoconiosis. When work history and current asbestos measurements were accounted, 9 workers who had more than 10 years of asbestos exposure history showed chest radiographic findings of pneumoconiosis with Finally, 4 workers showed finding of pulmonary fibrosis and/or pleural thickening at HRCT, and 2 of them had restrictive lung function changes. The study results showed that, among 139 sutjects, there were 4 (3%) definite asbeatosis cases confirmed with HRCT. The prevalence of probable asbestosis was 7% for 10-14 years of exposure, 13% for 15-19 years of exposure, and 23% for 20 or more years of exposure. The prevalence of compensable asbestosis with abnormal lung function was 4-6% for those with 15 or more years of exposure.
