The Effects of Particulate Matter on Respiratory Function of Schoolchildren in Jeju: A Panel Study

OBJECTIVE: To evaluate the changes in respiratory function associated with daily changes in particulate pollution.
METHODS
A total of 105 healthy schoolchildren of fourth and fifth grade elementary school age performed peak expiratory flow rates (PEFR) during 2 periods (May 14~June 15, Oct 22~Nov 23). We monitored concomitant ambient particulate matter (PM) levels during the periods.
RESULTS
An elevation PM(10) level of 1 microgram/m) led to decreases in PEFR of 0.20 L/min. A 1 microgram/m3 elevation of PM(2.5) led to a decrease of 0.36 L/min in PEFR. In addition, 1 day lag was associated with PM(10) levels and PEFR changes, and PM(2.5) showed the same result. The metal levels determined at PM(10) and PM(2.5) were found to be associated with decreased respiratory function.
CONCLUSIONS
An association between respiratory function and particulate pollution was observed when particulate pollutant levels were lower than the yearly average National Ambient Air Quality Standard in the Korea(50 microgram/m3).

Health Evaluation of ex-workers occupationally exposed to Carbon disulfide: Subjective Symptoms and Related Factors according to compensation for CS2 poisoning

OBJECTIVES
To investigate the chronological subjective symptoms and related factors in ex-workers who were occupationally exposed to Carbon disulfide.
METHODS
One hundred and seventy-seven subjects (147 males and 30 females) were randomly selected among ex-workers and interviewed by well trained doctors, who filled out a structured questionnaire developed by authors on the subjective symptoms. The questionnaire was composed of symptoms in the head and neck, and those of the neuro-psychological, respiratory, endocrine, musculoskeletal, and reproductive systems. Other factors such as general and occupational characteristics including work department, work duration, duration since cessation of exposure, and compensation for occupational disease were also investigated.
RESULTS
The mean subject age was 55.4 years. The mean cumulative exposure index (CEI) was 38.96 +/- 31.18. CEI was significantly higher in the compensated group (47.61 +/- 32.51) than in the non-compensated group (26.81 +/- 24.75)(p=0.000). Symptom complaints were significantly higher in the compensated group in all organ systems, and their incidence increased gradually until 10 years after cessation of work, and then decreased. The mean duration for cessation of exposure at the time of compensation was 10.97 years. The significantly different factors between the compensated group and the non-compensated group were total cumulative exposure index and the time interval between the cessation of work and the time of compensation.
CONCLUSION
Our findings suggest that total cumulative exposure index and the time interval between the cessation of work and the time of compensation may be important factors affecting the compensation for occupational disease. Therefore, further studies will be necessary.

The effect of Residential Environments on the Bronchial Hyperresponsiveness

OBJECTIVES
The increased prevalence of asthma over the last 30 years has been reported from many different countries, including Korea. The increased prevalence may be due to increased exposure to indoor air pollutants.
METHODS
In this study, the relationship between residential environments and airway hyper-responsiveness has been examined among 280 family members. The bronchial hyper-responsiveness was expressed as the log-transformed slope of FEV decrease over the log-transformed dose of methacholine concentration(BRindex). Simple regression analysis was done for the effect of sex, age, atopic status, and residential environments including active and passive smoking exposures on airway responsiveness. Based on simple regression results, multiple rwegression analysis was performed for total group and also according to sex.
RESULTS
The mean of BRindex of residents living in gas boiler heating houses was higher than that of central heating system (p=0.014). Smoking status was a significant determinant of bronchial hyper-responsiveness in both genders; male(p=0.017), female (p%.022). In the male group the mean of BRindex of current smokers was significantly higher than those of ex-smokers and non-smokers, and the mean of BRindex of passive smokers was higher than that of non-smokers, but statistical significance was border-line(p=0.069). In female group, the mean of BRindex of kerosene stove users was higher than that of non-users(p=0.057).
CONCLUSIONS
This study suggests that indoor air pollutants including type of heating, passive smoking and kerosene stove use can contribute to the increase in asthma prevalence in Korea.
