Establishment and operation of a cooperative program to identify work-related acute myeloid leukemia in a general hospital

Background

The purpose of this report is to introduce the occupational cancer surveillance system, implemented in June 2018, and to share the results of our cooperative program.

Methods

The cooperative program begins when the patient is diagnosed with acute myeloid leukemia (AML). Newly diagnosed AML patients are admitted to the internal medicine hematology department, then attending hematology physician requests a consultation from the occupational and environmental medicine (OEM) department. The OEM doctor next visits the hospitalized patient and interviews them to take their occupational history, and preliminarily evaluates the likelihood that the condition is associated with occupation. If the patient wants to apply for compensation through the Korea Workers' Compensation & Welfare Service, the patient was informed to visits the outpatient clinic of the OEM department and requests a ‘work-relatedness evaluation report’ for use in applying for compensation.

Results

Among the 103 patients, who received an OEM departmental work history evaluation, 18 patients were considered to have a work-related incidence and 12 patients were registered in the Industrial Accident Compensation Insurance system.

Conclusions

The present report provides data on a sustainable model for identifying occupational disease in a general hospital setting, while also informing patients about their occupational rights.

Comparison of risk-assessment tools for cardio-cerebrovascular diseases (CVD) in male shipyard workers: a cross-sectional study

Background

Periodic revision of assessment tools is essential to ensure risk assessment reliability and validity. Despite the recent revision of the Korea Occupational Safety and Health Agency (KOSHA) 2018, there is no evidence showing that the revision is superior to other cardio-cerebrovascular diseases (CVDs) risk-assessment tools for workplace health management. We conducted a comparative analysis using the Framingham risk score (FRS) as a gold standard to identify the most relevant CVDs risk-assessment tool for workplace health management.

Methods

We included 4,460 shipyard workers who had undergone a workers' health examination during January–December 2016. Risk levels for CVDs were calculated based on the FRS, KOSHA 2013, KOSHA 2017, KOSHA 2018 (2 methods), National Health Screening Program health risk appraisal (NHS HRA) 2017, and NHS HRA 2018. Study participants were categorized into low-risk, moderate-risk, or high-risk groups. Sensitivity, specificity, correlation, and agreement of each risk-assessment tool were calculated compared with the FRS as a gold standard. For statistical analyses, Spearman's rank correlation coefficient and the linearly weighted kappa coefficient were calculated.

Results

Sensitivity of the risk assessments was highest in the KOSHA 2018 (health risk appraisal [HRA]). The FRS showed correlation coefficients of 0.354 with the KOSHA 2013, 0.396 with the KOSHA 2017, 0.386 with the KOSHA 2018, 0.505 with the KOSHA 2018 (HRA), 0.288 with the NHS HRA 2017, and 0.622 with the NHS HRA 2018. Kappa values, calculated to examine the agreement in relation to the KOSHA 2013, KOSHA 2017, KOSHA 2018, KOSHA 2018 (HRA), NHS HRA 2017, and NHS HRA 2018 with the FRS, were 0.268, 0.322, 0.352, 0.136, 0.221, and 0.559, respectively.

Conclusions

The NHS HRA 2018 risk calculation method is a useful risk-assessment tool for CVDs, but only when appropriate classification criteria are applied. In order to enhance the risk-group identification capability of the KOSHA guideline, we propose to apply the classification criteria set in this study based on the risk group definition of the 2018 Korean Society of Hypertension guidelines for the management of hypertension instead of the current classification criteria of the KOSHA 2018.

Residency programs and the outlook for occupational and environmental medicine in Korea

Objectives

This study investigated the implementation of training courses and the overall outlook for occupational and environmental medicine (OEM) in Korea. We described the problems facing OEM residency programs in Korea, and reviewed studies dealing with the specialty of occupational health in developed countries in order to suggest directions of improvement for the OEM training courses.

Methods

We surveyed 125 OEM residents using a questionnaire in August 2012. A total of 23 questions about the training environment, residency programs, preferred institutions for post-licensure employment, and the outlook for OEM specialists were included in the questionnaire and analyzed according to the type of training institution and residency year. Responses from 88 residents (70.4 %) were analyzed.

Results

The major responsibilities of OEM residents were found to vary depending on whether they were trained in research institutes or in hospitals. OEM residents had a lower level of satisfaction with the following training programs: toxicology practice (measurements of biological markers, metabolites, and working environments), and OEM practice (environmental diseases and clinical training involving surgery). When asked about their eventual place of employment, OEM residents preferred institutions providing special health examinations or health management services. OEM residents reported a positive outlook for OEM over the next 5 years, but a negative outlook for the next 10 years.

Conclusions

Although a standardized training curriculum for OEM residents exists, this study found differences in the actual training courses depending on the training institution. We plan to standardize OEM training by holding a regional conference and introducing open training methods, such as an open hospital system. Use of Korean-language OEM textbook may also reduce differences in the educational programs of each training institution. Toxicology practice, environmental diseases, and clinical training in surgery are areas that particularly need improvement in OEM residency training programs.

The Effect of Participatory Approach Program for Cardiovascular Disease Prevention in the Workplace

OBJECTIVES
The purpose of this study was to develop a program applying participatory action-oriented training (PAOT) methods for the management of cardiovascular disease risk factors and to assess its effectiveness in the workplace.
METHODS
The cardiovascular disease risk factors of 817 workers in the workplace were assessed, and periodic health examination was conducted. Sixty-four workers (7.8%) were selected as a moderate-risk group and high risk group, and performed the participatory approach program for cardiovascular disease preventive management.
RESULTS
The sixty-four workers who participated in the participatory approach program made a total of 246 action plans, 3.8 action plans per person and 115 action plans were achieved after three months, so that 46.7% of action plans have been achieved. In the intervention group that participated in the program, the rate of smoking decreased from 36.2% to 8.6% and systolic and diastolic blood pressure significantly decreased from 140.0+/-9.4 mmHg to 134.2+/-12.3 mmHg and from 92.6+/-7.1 mmHg to 80.0+/-9.8 mmHg respectfully. In addition, risk factor score also decreased by 0.3, whereas the control group who did not participate in the program also showed a significant reduction in the mean systolic and diastolic blood pressure. Regular exercise increased from 8.5% to 16.0%.
CONCLUSIONS
To improve the effectiveness of the participatory approach program for the management of cardiovascular disease risk factors, the need to continues implementing programs and analysis of the long-term effects are required.

Comparison of Cardiovascular Disease Status Between Large Scale Industry Office and Self Employed Male Workers

OBJECTIVES
The aim of this study is to compare the cardiovascular disease status between large scale industry office and self employed male workers who have gaps in their intensive health management.
METHODS
The cross sectional study was carried out with subjects composed of 244 shipyard office male workers and 381 self employed male workers, aged 30 to 55 years. They were given a health exam in a general hospital from January 2007 to June 2009. Information was based on data from self-administered questionnaires, physical examination and laboratory results on blood samples. The degree of coronary artery calcification and stenosis was evaluated by a 64 channel multi-detector computed tomography angiography.
RESULTS
The prevalence of hypertension(p<0.01) and diabetes mellitus(p=0.02) and waist circumference(p<0.01) was significantly lower in the shipyard office workers than in self employed workers. Proper drinking(p<0.01) was more commmon in shipyard office workers. The rates of coronary artery calcification and stenosis were estimated to be 18.4% and 11.5% respectively for shipyard office workers and were significantly lower than the 26.5% and 21.5% for self employed workers. After adjustment for age, the probability of coronary artery stenosis in shipyard office workers was lower than in self employed workers (OR=0.56, 95% CI=0.35~0.90).
CONCLUSIONS
The study results verified the benefits of a health promotion and intervention program and the healthy worker effect for cardiovascular diseases. More specifically, it verified the benefits regarding coronary artery stenosis in large scale industry office workers. This result should encourage the establishment of health promotion programs and the study of occupational epidemiology.

The Effectiveness of an Intervention Program for Subway Operators for Symptoms of Post-traumatic Stress Disorder After the Persons Had Fallen Under a Train

OBJECTIVE: The aims of this study were to evaluate the level of job stress in subway workers and to determine the main factors that aggravated their PTSD(Post-Traumatic Stress Disorder) symptom, which were due to having fallen under a train, and to verify the effectiveness of the intervention program.
METHODS
The questionnaires utilized in the survey were composed of the Korean Occupational Stress Scale (the short form, 24 items), the depression scale, the coping scale II, the PTSD scale and the life style, health status and occupational properties. The intervention program for PTSD symptoms was executed for 18 subway operators who had experienced deadly accidents in their service from June 2004 to May 2006. The intervention programs included exercise, meditation and deep breathing for 8 weeks and a written message was sent twice per week.
RESULTS
Compared with pre-intervention, the post-intervention group showed significantly lower scores for the KOSS, depressive symptom and the PTSD.
CONCLUSION
This study showed the intervention program was effective to lower the job stress, the PTSD symptoms and depressive symptoms, and to improve the coping skills. It was suggested that the mental health promotion program should be implemented for subway workers to prevent occupational mental diseases.

A Workplace Cardiovascular Health Promotion Program and its Short-term Health Effects

OBJECTIVES
Cardiovascular disease is the leading cause of death and a major source of workers' compensation claims in Korea. Since 2000 the Korea Occupational Safety and Health Agency (KOSHA), working through local occupational health institutions, has supported cardiovascular disease prevention programs at a number of companies in Korea. The purpose of this study was to assess the short-term effects of this effort.
METHODS
A total of 11,077 workers at risk were enrolled in the workplace cardiovascular disease prevention program and 5,902 workers (53.3%) completed the 1-year course during 2007. The program consisted of a medical checkup and health counseling for the workers by occupational health nurses. The guidelines for this prevention program were adopted from KOSHA Code H-11-2004. To determine the program's effectiveness, the workers' risks for cardiovascular disease were assessed before and one year after completion of the program.
RESULTS
The intervention led to significant reductions in the mean systolic and diastolic blood pressures of 4.9 mmHg and 3.1 mmHg, respectively. Mean total cholesterol and BMI were also reduced significantly by 8.4 g/dl and 0.1 kg/m2. The rate of smoking was decreased by 6.0% and the percentage of workers engaging in regular exercise was increased by 23.1%. Of the 3,530 workers with the low risk and above, the overall cardiovascular risk was improved in 1,734 (49.1%) of them.
CONCLUSIONS
The cardiovascular disease prevention program supported by the Korea Occupational Safety and Health Agency reduces cardiovascular diseases risks among workers and may improve the health status of workers in Korea.

The Need-Assessment of Employee Assistance Programs among Korean Manufacturing Industry Workers in Gyeongsang-do

OBJECTIVE: Employee Assistance Programs have recently been introduced to Korea as a potential solution to relieve workers's stress and improve their mental health. This study was conducted to determine the needs of EAPs among manufacturing industry workers in Gyeongsang-do Korea.
METHODS
Responses to structured survey questionnaires were received from 556 workers at 7 mediumsized manufacturing factories.
RESULTS
Among the 556 workers who responded to the questionnaire 'Prevention & management of work stress', 'Mental health & management of daily life stress', 'Physical exercise health promotion program'. 'Elderly care', 'Outplacement', 'Financial support for leisure activities', and 'Ensuring leisure time'were raised as the main issues and services that they expected to be provided by the company.
CONCLUSION
EAPs have recently been introduced in Korean companies. The study results will be helpful to clarify the needs of individual Korean workers and to determine what EAP services should be provided.

Effect on Active Exercise Programs in Employees with Chronic Low Back Pain

OBJECTIVE: To examine the relative efficacy of three active exercise programs for work-related, chronic low back pain, and to observe to what extent the programs affected the mechanical stability of the lumbar region.
METHODS
The subjects were 64 employees who were randomly divided into three groups to match the three active exercise programs which were performed 3 times a week for 6 months. All subjects were assessed with the same measurements at a pre-study examination, and then were reassessed at 2 weeks, 3 months and 6 months after the study.
RESULTS
The pain intensity didn't show any significant difference among the three groups. However, the Oswestry Disability Index showed significant differences among the three groups at 6 months and the lumbar and thoracic exercise groups showed significant decreases compared to the general physiotherapy group (p<0.05). Maximal stretching with both hands in the overhead direction showed a significant difference among the three groups at 3 months and 6 months, and the thoracic exercise group at 6 months showed a significant increase in overhead stretching compared to the lumbar exercise and general physiotherapy groups (p<0.05). The lumbar region angle of inclination showed significant differences among the three groups at 2 weeks, 3 months and 6 months, with the thoracic exercise group being decreased significantly more than the lumbar exercise and general physiotherapy groups at 6 months (p<0.05).
CONCLUSIONS
Exercise aimed at increasing thoracic mobility has an effect on lumbar stability. Furthermore, it is far more effective for lumbar stabilization than general physiotherapy and deep muscle strengthening lumbar exercise.

Validity of the Energy Expenditure Prediction Program to Evaluate Energy Expenditure During Work

OBJECTIVES
The Energy Expenditure Prediction ProgramTM (EEPP) has been considered as a simple and quantitative method to evaluate physical work load. However, the adoption of EEPP directly to Korean workers is problematic because it was developed in a laboratory setting for Caucasians. Therefore, this study was conducted to validate EEPP for Korean workers.
METHODS
The study subjects consisted of 60 workers from two factories. Cycle ergometer test was conducted to calculate physical work capacity, and heart rate monitoring was conducted to check heart rate during work. After observing the task, energy expenditure was estimated by EEPP.
RESULTS
EEPP underestimated energy expenditure less than EEHR (energy expenditure checked by heart rate) did(p<0.0001). The factors effecting EEHR were EEPP and task type. After dividing the task into regular and irregular tasks, the irregular task had a larger difference between the values from the two methods. We provided task specific regression models between EEHR and EEPP.
CONCLUSIONS
Because EEPP underestimated energy expenditure, it needs to be adjusted before use with Korean workers. It is suggested that different adjusting equations are formulated for regular and irregular tasks. Further study to develop a specific energy estimation model appropriate for Koreans is needed to obtain more precise estimation.

Status of the Occupational Health and Safety Programs of Several Hospitals in Korea

OBJECTIVES
As health care workers suffer from various hazards in hospital, an occupational safety and health program is very important to them. This study was conducted to investigate the status of the occupational safety and health programs of several hospitals in Korea.
METHODS
We conducted a questionnaire survey on 941 hospitals in Korea. All questionnaires were posted to the department of general affairs at each hospital and responses were received from 108 hospitals (11.5%). Eight responses were excluded because of insufficient data and the remaining 100 questionnaires were analyzed. The collected data were evaluated using the SPSS 10.0 statistical package and the demands for occupational safety and health programs between two groups were analyzed by RIDIT analysis.
RESULTS
The study subjects were 53 general hospitals and 47 hospitals. An occupational safety and health committee, infection control committee, safety manager, health manager, activities for vaccination, reports for needle stick injury and records related to occupational injury were more likely to be present in general hospitals than hospitals (p<0.05). Demands for occupational safety and health were higher in general hospitals than hospitals. Especially, demands for measurement of working environment, improvement of ventilatory system, management of chemical materials and supply of protective equipment were significantly higher in general hospitals than hospitals (p<0.05).
CONCLUSIONS
We concluded from the findings of our study that occupational safety and health programs are not yet sufficient in Korean hospitals. So, we expect that special management, policies and laws for health care workers will be promulgated.

Characteristics of Work-Related Musculoskeletal Disorders and Effect of Intervention Program in Shipyard Workers

OBJECTIVES
This study was conducted to estimate the characteristics of work-related musculoskeletal disorders and the effect of intervention program in shipyard workers.
METHODS
Information on work related absences,due to musculoskeletal disorders, were collected between January, 1 2000 and August, 31 2002. Age, duration of work, job type, anatomical site of disease, cause and duration of absence due to sickness were investigated.
RESULTS
From a total of 158 cases there were 108(68.4%)and 50(31.6%)cases of back, and other musculoskeletal complaints,respectively. 55(34.8%)of the cases were caused by repetitive trauma and 103(65.2%)were due to non-repetitive causes. 90(83.3%)of the back complaint cases were due to non-repetitive causes, but there was a repetitive cause of other musculoskeletal complaints in 37(74.0%)cases. Back complaints were higher in workers carrying heavy materials, but other musculoskeletal complaints were higher in painting workers.Half of workers after an absence due to sickness had returned to work within 32.0 weeks and 25.4 weeks,in the cases of back and other musculoskeletal complaints, respectively. After the application of an intervention program for musculoskeletal disease, the incidence rate of absence due to work-related musculoskeletal disorders was reduced significantly.
CONCLUSION
The introduction of a musculoskeletal intervention program reduced absences due to work-related musculoskeletal disorders.

The Experience of Occupational Health Care Providers in the Government-funded Subsidized Occupational Health Program for Small Scale Industries: Use of Focus Group Interview

OBJECTIVE:This study was conducted to provide the fundamental information to prepare better strategies based on the experiences of occupational health care providers who are involved in the Government-funded subsidized occupational health programs for small scale industries.
METHODS
The focus group methodology was used in this study.The data was collected using focus group interviews and analyzed in the framework of the Grounded theory method as mapped by Strauss and Corbin(1990). The subjects enrolled in this study included 20 occupational health care providers in the Government-funded subsidized occupational health program for the SSI;6 doctors and 14 nurses.
RESULTS
We identified 69 concepts,28 sub-categories and 5 categories(Table 2).The categories were the Program's Goals, Various Services,Difficulties in Implement Services, Program Evaluation,and Suggestions for Alternative Plans.
CONCLUSIONS
This program showed that autonomous health management ability has not changed although the possibility for small scale industries to start the health management arose by acknowledging the importance of the health management in the work-place. A successful program should provided a practical service that is planned by the characteristics and the requirements of the workers,a better service than the standard, more visiting times,and a trusting relationship among workers. The occupational health agency's quality and the health manager's specialty needs to be improved and the health manager's authority should be consolidated. An actively cooperative triangular relation among the Labor Department/KOSHA, service organizations, and the workplace is required.In addition,alternative plans were suggested.These included various programs for planning an autonomous service suitable for the quality of the workplaces,a model set-up suitable for the quality of the workplaces,and a role model set-up for the health manager.

Need Assessment for Stage of Behavior Change in Workplace Health Promotion Strategy

OBJECTIVES
In order to evaluate the stage of behavior change for health promotion strategy in workplace METHODS: Questionnaire survey was carried out for 309 workers who employed in H Electronics company in Kyongin area from September to October 1998.
RESULTS
According to need assessment of four step such as precontemplation stage, contemplation stage, action stage, maintenance stage which were to change of behavior for health promotion, the percent for quitting smoking were observed 48.8%, 43.1%, 8.1%. 0 %, and for the restriction of drinking were 29.8%, 10.6%, 9.3%, 5.3%; for fitness 11.1%, 31.0%, 37.3%, 20.6% in male 26.6%, 27.8%, 26.6%, 19.0% in female, for regular diet 26.1%, 19.8%, 38.7%, 15.3% in male, 27.8%, 22.2%, 40.0%, 10.0 % in female and for stress management 5.8%, 10.6%, 8.7%, 75.0% in male, 13.6%. 22.7%, 19.7%, 44.0% in female. In heavy smoker (p=0.017) and heavy drinker, (p=0.021) the percent of precontemplation stage was significantly higher than in lower drinker and light smoker.
CONCLUSIONS
This study suggested that company support for health promotion in workplace was weak and workers of high risk group for smoking and drinking were in precontemplation stage. So before conducting the health promotion program, health risk appraisal and need assessment for stage of behavior change should be taken for the efficient heath promotion strategy.

The small scale industries in Korea are important in occupational health care management due to high accident rate and poor working condition. The authors surveyed the 4,811 small scale industries participated in the 1997 subsidiary program on health care management. The contents of survey were the general characteristics of the factory, the results of workers' general and occupational health examination, the results of workplace environmental monitoring, and the number of occupational accident and injured person at 1995 and 1996. We classified all industries by 67 groups. The prevalence of the general disease was higher in 'Manufacture of other fabricated metal productsj. rManufacture of textiles', 'Manufacture of basic metalsj. In occupational disease, that was higher in FManufacture of metal products', 'Manufacture of basic metals', so in environmental monitoring in 'Manufacture of textiles', 'Manufacture of other fabricated metal products' 'Printfrig' in occupational injuries in 'Manufacture of chemical products', 'Manufacture of basic metals'. However the factories of 'manufacture of motor vehicle', 'manufacture of machine and equipment', 'manufacture of electric product' in the program are large in magnitude, but less important in occupational health management. We observed that the prevalence of general disease, occupational disease, workplace environmental monitoring, occupational injuries of subsidized industries in 1996 were increased compared with those in 1995. and were larger than those of nationwide statistics. These phenomenon shows that the subsidiary health program for participated industries is important and to be urgent. We observed that the major groups in district were different and so the approaching method of each district must be various and different. And the health program manager should focus on the C grade in health examination and the action level in workplace monitoring more than the D grade and the permissible level.

To find the factors affecting reading agreement of pneumoconiotic findings of simple chest P-A films and to develop the definite quality assurance (Q.A) program for the reading of pneumoconiosis, I gathered the reading reports of the previously developed forty four chest films from nationwide thirty nine radiologists and compared those with the findings decided by the stepwise panel readings of the highly qualified eight radiologists. The reading agreement of profusion by complete or short classification was higher in the radiologists with sufficient reading experiences of pneumoconiosis during residency training. It was also higher in the radiologists with reference to ILO standard films or those working in the agencies for pneumoconiosis within one category of profusion by complete classification. Other factors were not related with the reading agreement of profusion. In small opacity and pleural thickening, there were not any consistent and meaningful findings between the reading agreement and radiologists' characteristics. From the above results, it is necessary to develop the Q.A program to improve the reading agreement of pneumoconiotic findings, especially small opacity and pleural lesions.

Relating Factors of Industrial Accident

This study was conducted to estimate the causing factors of industrial accident, and to prepare the fittable control program for accident through the evaluation of views of the patients who suffered from accidents. From January 5 to February 6 1993, questionnaire survey was executed on 166 persons who were injured and covered by the insurance under industrial accident compensation law at four university hospitals in Pusan. The results were as follows : 1. Most of subjects worked in the manufacturing (56.6%) and construction(29.5%), and in the small factory less than 49 workers(56.0%). They were thirties and forties(63.8%), less than 3 years tenure(53.6%), habitual drinker(69.2%). And, 72.3% worked for above 9 hours per day, 53.6% worked as near clerks, 89.2% thought that work is rather suitable to his own personality. 2. Self-recognition for the causing factor of industrial accident was the other's malfunction(19.3%), carelessness(15.1%), lack of education for safety(15.1%), and they thought the accident occurred by mainly some defect of establishment and working environment(34.9%), unskillfulness(22.9%), self-carelessness(22.3%). 3. The interests and recognition on safety was insufficient, 88.6% require job education, but 53% pointed out the necessity for strengthening of present status-rather incomplete with relying on self-education (40.9%), no previous participation in safety education (29.5%), etc. 4. For the kind of accident, fall-down(16.3%), crash(13.3%), falling(12.7%) was frequent, head(24.7%) and fracture(41.0%) was frequent for the part and type of injury, respectively. And, they had more injured by cooperative work (82.5%) than single work. As results, improvement of working environment and strengthening education program would be emphasized for the effective control of industrial accident. And, there should be prepared the specific prevention program for the small scale industries.

Lung Function Test for Workers and its Quality Control Program

Lung function test can be influenced by technical as well as biological variables, and the quality control of test has been stressed as an important part of lung function test program. This study was done to survey the current status of lung function test program in workers' health examination, and examine the variablity of test results when quality control is not achieved. Forty nine Workers' Health Examination Institutes and 7 Pneumoconiosis Referal Centers were surveyed for their current lung function tests, types of spirometers, and performance of technicians. To examine the effect of quality control program of lung function tests on the variablity of test results, 118 miners were tested under a quality control guideline and the results were compared to those obtained ignoring 4 major areas of the guideline-spirometer calibration, minimum number of tests per session, suitable prediction equations, and selection process of final results for each tested subjects. Among surveyed Institutes and Centers, only 38%(20/53) are performing any calibration of spirometers, and most of those calibrations are done automatically by the machine. All the surveyed Institutes and Centers obtain acceptable test no more than 2 times. The background of technicians include office workers and nurse aids without an adequate training in pulmonary physiology, and the testing posture and formats vary among Institutes and Centers. When the types of pulmonary dysfunction under the definition of the Ministry of Labor were compared between those obtained with and without quality control guidelines, spirometer calibration affected the test results in 6(5.1%) out of 118 subjects. The change in the minimum number of adequate tests from 1 to 3 and more brought changes in 19(16.1%) subjects. The test results of 9(7.6%) subjects differed between two different prediction equations, and the results of 7(5.9%) subjects differed between two different selection process of final results. The current practice of lung function tests of workers needs quality control programs, and the results of this study indicate that the first remedy should be to bring the minimum number of acceptable test to more than 3.
